摘要
目的了解肿瘤患者分离鲍曼不动杆菌的分布及耐药情况,为临床合理使用抗菌药物预防和控制感染提供依据。方法收集我院肿瘤专科2014年1月1Et~2015年12月31日各类临床标本鲍曼不动杆菌分离株208株.检测鲍曼不动杆菌对常用的15种抗菌药物的敏感性。结果208株鲍曼不动杆菌中,分离自痰液和支气管肺泡灌洗液59.14%(123/208),分离自脑脊液8.65%(18/208),分离自尿液10.58%(22/208),分离自血液8.17%(17/208),分离自伤口分泌物8.65%(18/208),分离自胸腹水4.81%(10/208)。鲍曼不动杆菌对哌拉西林、氨苄西林/舒巴坦、美洛培南、环丙沙星、左旋氧氟沙星、头孢吡肟、头孢他啶、庆大霉素、妥布霉素、哌拉西林/他唑巴坦的耐药率较高,依次为91%、84%、76%、82%、83%、78%、75%、81%、50%、81%;阿米卡星、头孢哌酮/舒巴坦、米诺环素、替加环素、多粘菌素B的敏感率高达75%~98%。结论鲍曼不动杆菌对哌拉西林、氨苄西林,舒巴坦、美洛培南、环丙沙星、左旋氧氟沙星、头孢吡肟、头孢他啶、庆大霉素、妥布霉素、哌拉西林/他唑巴坦的耐药率较高;而对阿米卡星、头孢哌酮,舒巴坦、米诺环素、替加环素、多粘菌素B的敏感率高。治疗时应根据药敏结果合理选择药物。
Objective To explore the distribution and drug resistance conditions of isolate Acinetobacter baumannii in cancer patients, and to provide basis for rational use of antibacterial agents to prevent and control infections. Methods 208 isolates of Acinetobacter baumannii were collected from various clinical samples from department of tumor in our hospital during January 1, 2014 to December 31, 2015. The sensitivities of Acinetobacter baumannii towards 15 com- monly-used antibacterial agents were detected. Results 59.14%(123/208) of the 208 isolates of Acinetobacter bauman- nii were collected from sputum and bronchoalveolar lavage fluid, 8.65%(18/208) were from cerebrospinal fluid, 10.58% (22/208) from urine, 8.17%(17/208) from blood, 8.65%(18/208) from wound secretion, and 4.81%(10/208) from hy- drothorax and ascites. The drug resistances of Acinetobacter baumannii to piperacillin, ampicillin/sulbactam, meropen- em, ciprofloxacin, levofloxacin, cefepime, ceftazidime, gentamicin, tobramycin, and piperacillin/tazobactam was relatively high, which was 91%, 84%, 76%, 82%, 83%, 78%, 75%, 81%, 50%, and 81%, respectively. The sensitivities of Aeinetobacter baumannii towards amikacin, cefoperazone/sulbactam, minocycline, tigecycline, and polymyxin B were as high as 75%-98%. Conclusion The drug resistance of Acinetobacter baumannii to piperacillin, ampicillin/sulbactam, meropenem, ciprofloxacin, levofloxacin, cefepime, ceftazidime, gentamicin, tobramycin, and piperacillin/tazobactam was relatively high, and its sensitivities towards amikacin, cefoperazone/sulbactam, minocyeline, tigecycline, and polymyxin B were high. Agents should be reasonably selected based on the results of drug sensitivity.
