2型糖尿病患者碘营养与甲状腺功能异常的研究 被引量:3

摘要 目的调查2型糖尿病患者的碘营状态,探讨碘营养对2型糖尿病合并甲状腺功能异常的影响。方法回顾性分析520例门诊及住院2型糖尿病患者临床资料(为研究组),采用酸消化砷-铈接触法检测尿碘,以化学发光法检测甲状腺激素。另收集来医院体检的正常健康人200例设为对照组。结果 2型糖尿病患者尿碘中位数为179.2μg/L,严重碘缺乏、中度碘缺乏、轻度碘缺乏、碘适量、碘超足量、碘过量的构成比分别为:0.6%(3/520)、8.1%(42/520)、12.9%(67/520)、46.0%(239/520)、21.3%(111/520)、11.2%(58/520),研究组与对照组间的尿碘中位数及尿碘频率分布无统计学差异;研究组甲状腺功能减退患病率为4.2%,高于对照组(2.0%),比较有统计学差异,而甲状腺功能亢进及低T_3综合症患病率两者相比无统计学差异;Logistic回归分析显示女性、TPO抗体阳性、碘过量与甲状腺功能减退发生呈正相关。结论 2型糖尿病患者的碘营养总体在适宜水平,2型糖尿病患者合并甲状腺功能减退患病率较高,2型糖尿病患者应避免过量碘摄入。 Objective To investigate the iodine nutrition in type 2 diabetes mellitus patients,and further to discuss the effect of iodine nutrition on thyroid function in patients with type 2 diabetes mellitus. Methods Retrospective analysis of clinical data was carried out among 520 cases with type 2 diabetes mellitus,the urinary iodine level was measured by arsenic- cerium contact process method,the levels of thyroid hormones were measured by Chemiluminescence method. Results The median of urinary iodine of the type 2 diabetes mellitus patients was 179. 2 ug / L. The proportion of severe iodine deficiency,moderate iodine deficiency,mild iodine deficiency,appropriate amount of iodine,more than adequate iodine and iodine excess were 0. 6%( 3 /520),8. 1%( 42 /520),12. 9%( 67 /520),46. 0%( 239 /520),21. 3%( 111 /520) and 11. 2%( 58 /520). There were no significant difference in the median urinary iodine as well as the urinary iodine distribution between type 2 diabetes mellitus patients and healthy people. The prevalence of hypothyroidism in type 2 diabetes mellitus patients was higher than the healthy people( 4. 2% vs 2. 0%,P〈0. 05). There was no significant difference in hyperthyroidism and low FT3 syndrome between the two groups. Logistic regression analysis showed that female,TPO antibody positive and iodine excess were positively correlated with hypothyroidism. Conclusion The iodine nutrition of type 2 diabetes mellitus patients was in a suitable level. The prevalence of hypothyroidism in type 2 diabetes mellitus patients was higher than healthy people.Type 2 diabetes mellitus patients should be avoid excessive iodine intake.
