摘要
目的应用磁共振扩散峰度成像(DKI)研究轻度认知功能障碍(MCI)患者及正常老年人海马的差异。方法搜集23例MCI患者为病例组(MCI组),23名健康志愿者为正常对照组(HC组),分别测量双侧海马的平均峰度(MK)值、轴向峰度(Ka)值、径向峰度(Kr)值、各向异性分数(FA)值、平均扩散(MD)值、轴向扩散(Da)以及径向扩散(Dr)值,使用两独立样本t检验对两组间所测DKI各参数进行分析;使用受试者工作特征曲线(ROC)曲线下面积(AUC)评价上述各参数值诊断MCI的能力;使用Pearson相关性分析对上述海马的DKI各参数值与MMSE评分的相关性进行分析。结果与HC组相比,MCI组双侧海马的MK值、Kr值、FA值、Da值、Dr值减低,差异具有统计学意义(P<0.05),双侧海马的MD值增加,差异具有统计学意义(P<0.05),双侧海马的Ka值比较差异无统计学意义(P>0.05);ROC曲线显示,左侧海马MD值的AUC最大,为0.765;Pearson相关性分析结果,双侧海马的MK值、Kr值、FA值、MD值、Da值、Dr值与简易精神状态检查量表(MMSE)评分均具有相关性(P<0.05),海马的Ka值与MMSE评分无相关性(P>0.05)。结论 DKI技术能够敏感地探测到MCI患者海马的变化,为MCI患者和正常对照者的鉴别提供有效的影像学方法。
Objective To investigate the value of diffusion kurtosis imaging( DKI) in the microstructure of hippocampus of the patients with mild cognitive impairment( MCI) and healthy controls. Methods 23 patients with mild cognitive impairment and 23 healthy controls( HC) were collected,the mean kurtosis( MK),axial kurtosis( Ka),radial kurtosis( Kr),fractional anisotropy( FA),mean diffusivity( MD),axial diffusivity( Da) and radial diffusivity( Dr) values were measured in hippocampus,two independent sample t-test were used to analyze the parameters of DKI between the two groups; Receiver operating characteristic( ROC) test were used to assess the ability of the above DKI parameters to diagnose MCI; Pearson correlation analysis were performed between hippocampal DKI parameters and Mini- mental state examination( MMSE) scores. Results Compared with HC,the MK value,the Kr value,the FA value,the Dr value in hippocampus was decreased in MCI,and the differences were statistically significant( P〈0. 05),the MD value was increased in MCI,and the differences were statistically significant( P〈0. 05),the Ka values between the two groups showed no significant difference( P〈0. 05); Area under ROC curve showed that the biggest area value of 0. 765 belongs to the MD value in the left hippocampus; Pearson correlation analysis showed that there was correlation between MK values,Kr values,FA values,Da values,Dr values and MMSE scores( P〈0. 05),there was no correlation between Ka value and MMSE score( P〈0. 05). Conclusion DKI technique was able to detect the changes of hippocampus in MCI patients,providing an effective imaging method for the identification of MCI patients and healthy controls.
