摘要
目的探讨抗甲状腺过氧化物酶抗体(TpoAb)和抗甲状腺球蛋白抗体(Tg Ab)在甲状腺疾病诊断中的临床价值。方法选取2011年10月至2015年9月甲状腺疾病患者357例,同期体检健康者50例(正常对照组),根据疾病诊断将患者分为两组(A组和B组),A组为非自身免疫性甲状腺疾病(单纯性甲状腺肿、甲状腺肿瘤、甲状腺功能亢进、原发性甲状腺功能减退症),B组为自身免疫性甲状腺疾病(Graves病,桥本病),对比观察两组以及两组与正常对照组TgAb和TpoAb含量的差异。然后将各疾病作为亚组,对比各疾病间TgAb和TpoAb测定结果间的差异。结果 B组两种抗体阳性率、测定结果均显著高于A组;单纯性甲状腺肿、甲状腺腺瘤血清TgAb和TpoAb测定结果与正常对照组比较,差异无统计学意义(P>0.05);分化型甲状腺癌、甲状腺功能亢进、原发性甲状腺功能减退症、Graves病、桥本病患者的TgAb水平及TpoAb水平均高于正常对照组(P<0.05)。桥本病患者TpoAb水平、Graves病患者的Tg Ab水平均明显高于其他组(P<0.05)。分化型甲状腺癌患者TgAb和TpoAb阳性率高于甲状腺腺瘤,差异有显著性(P<0.05)。结论血清Tg Ab、TpoAb水平对甲状腺疾病的诊断具有重要价值。
Objective To discuss the clinical value of TpoAb and TgAb in the diagnosis of thyroid diseases. Methods 357 cases of thy-roid disease and 50 cases of healthy people,from October 2011 to September 2015,were selected in this study. And patients with diseases were divided into two groups（group A and group B）,group A with autoimmune thyroid diseases（simple goiter,thyroid tumor,hyrerthyroidism,prima-ry hypothyroidism）,group B with autoimmune thyroid disease（ Graves,hashimoto′s disease）. The differences of TgAb and TpoAb between groups. Results Two antibody positive rate of Group B were significantly higher than those of group A;Serum TgAb and TpoAb of simple goiter or thyroid adenoma has no significance difference compared with normal control group（ P 〉 0. 05）. TgAb,TpoAb levels of differentiated thyroid cancer,hyperthyroidism,primary hypothyroidism,Graves′disease,hashimoto′s disease were higher than those in normal control group（ P 〈0. 05）. The TpoAb level in Hashimoto′s disease and TgAb level in Graves′disease were significantly higher than other groups,respectively（ P 〈 0. 05）. TgAb and TpoAb positive rate in patients with differentiated thyroid cancer were higher than those of the thyroid adenoma（ P 〈 0. 05）. Conclu-sion Serum TgAb,TpoAb is of important clinical value in the diagnosis of thyroid diseases.
