摘要
目的 探讨CD147和血管内皮生长因子(vascular endothelial growth factor,VEGF)在肾细胞癌(renal cell carcinoma,RCC)中的表达及其与临床病理特征的关系。方法 采用免疫组化PV6000法检测97例RCC和30例正常肾组织中CD147和VEGF的表达,分析二者表达与RCC临床病理特征的关系。结果 CD147和VEGF在RCC中的表达均明显高于正常肾组织(P均〈0.01);CD147和VEGF表达与肿瘤大小、病理分级、TNM分期、浸润性生长和转移之间呈显著相关(P均〈0.05);与患者性别、年龄、组织学类型及伴肉瘤样分化之间无显著相关性(P均〉0.05)。RCC组织中CD147和VEGF的表达呈正相关(rs=0.631,P〈0.01)。41例获得随访结果,随访时间6~144个月,其中死亡7例,11例发生肺、肝、骨、脑、对侧肾、肾上腺、皮肤和卵巢等部位远处转移。结论 联合检测RCC中CD147和VEGF的表达,有助于综合探讨RCC的发病机制。CD147和VEGF可以作为新的预测RCC浸润、转移和判断预后的重要因子,并为临床基因靶向治疗提供新的理论依据。
Purpose To investigate the expression of extracellular matrix metalloproteinase inducer (CD147) and vascular endothelial growth factor (VEGF) in renal cell carcinoma (RCC) and their relationships with clinicopathological characteristics of RCC, and to explore the biological markers for prediction of tumor behaviour and prognosis, and to provide novel theoretical way for RCC treatment. Methods The expression of CD147 and VEGF was detected by PV 6000 immunohistochemistry in 97 cases of RCC and 30 cases of normal renal samples. The expression of CD147 and VEGF in RCC and their relationships with clinicopathological characteristics and prognosis of RCC was also analyzed. Results The expression of CD147 and VEGF in RCC was significantly higher than that in normal renal samples (P 〈 0.01 ). Higher expressive levels of CD147 and VEGF were associated with tumor size, WHO/ISUP grade, TNM staging, tumor invasion and metastasis ( P 〈 0.05 ) , but not related to gender, age, histological classification, with or without sarcomatoid differentiation (P 〉0. 05). Positive correlation was found between the expression of CD147 and VEGF in RCC (rs =0. 631, P 〈 0. 01 ). 41 patients were followed up for 6 to 144 months after the surgery. Among them, 7 patients died and 11 patients developed dis- tant metastasis to lung, liver, bone, brain, contralateral kidney, adrenal, skin or ovarian. Conclusion Combined detection of CD147 and VEGF in RCC may useful for exploring pathogenesis of RCC. They could be used as novel biological markers for prediction of tumor invasion, metastasis and prognosis. They may provide the theoretical basis and new potential therapeutic targets for RCC treat- ment.
