摘要
目的探讨降钙素原(PCT)联合C反应蛋白(CRP)和IL-6在小儿感染性肺炎诊断中的应用效果。方法选择2012年6月至2015年6月我院收治的儿科住院患儿和健康体检儿童为研究对象,抽取静脉血液进行PCT、CRP和IL-6含量检测。比较肺炎组与对照组三种物质检测结果,比较不同肺炎类型的三种物质的检测结果,统计患儿肺炎阳性检出率。结果肺炎组与对照组血清学指标比较,PCT、CRP和IL-6含量在两组之间的差异均存在统计学意义(P〈0.05)。肺炎组与对照组组内血清学指标相关系数比较,PCT与CRP、PCT与IL-6、CRP与IL-6进行组内相关分析,均呈现正相关关系。对照组间则不存在相关关系。两组患儿血清学指标比较,PCT、CRP和IL-6在细菌性肺炎组和非细菌性肺炎组之间的差异有统计学意义(P〈0.05)。三项血清学指标对不同类型肺炎阳性率比较,细菌性肺炎组的PCT、CRP、IL-6及三者联合阳性检出率明显高于对照组,两组指标检查差异有统计学意义(P〈0.05);细菌性肺炎组PCT、IL-6阳性率与非细菌性肺炎组之间的差异有统计学意义(P〈0.05),而CRP指标在两组之间的差异无统计学意义(P〉0.05)。结论联合检测PCT、CRP和IL-6在小儿感染性肺炎的早期诊断中具有重要意义。
Objective To investigate procalcitonin (PCT) and C reactive protein (CRP) and interleukin 6 (IL-6) in the diagnosis of pneumonia in children with infection. Methods 174 children from June 2012 to June 2015 in our hospital were selected as the research object. The extraction of venous blood was test for PCT, CRP and IL-6 content detection. Three substance detection results were compared with the control group. Detection results of different types of pneumonia were compared in different group. And positive rate of pneumonia detection in children were compared in each substance detection. Results Serum levels of PCT, CRP and IL-6 were significantly different between pneumonia group and control group ( P 〈0.05). In pneumonia group correlation of serum markers of PCT and CRP, PCT and IL-6, CRP and IL-6 were analyzed, which showed positive correlation. There was no correlation in control groups. Concentration of PCT, CRP, IL-6 and combination of the three which were detected in bacterial pneumonia group was higher than control group, differences showed statistical significance ( P 〈0.05). Concentration of three indexes between bacterial pneumonia group and non bacterial pneumonia group were compared, and the differences of PCT, CRP and IL-6 were statistically significant ( P 〈0.05). The differences of PCT and IL-6 positive rate between the bacterial pneumonia group and non bacterial pneumonia group showed statistical significance ( P 〈0.05), while the difference of CRP index between the two groups had no statistical significance ( P 〉0.05). Conclusions Combined detection of PCT, CRP and IL-6 in the early diagnosis of children with infectious pneumonia has great significance.
