摘要
目的:观察中药外洗方2号联合甲氨蝶呤片治疗类风湿性关节炎(RA)寒湿痹阻型的治疗效果。方法:将88例RA寒湿痹阻证患者按数字表法随机分为对照组和治疗组,两组均44例;对照组口服甲氨蝶呤片治疗,疼痛剧烈时采取美洛昔康片内服;治疗组在对照组用药基础上给予中药外洗方2号熏洗治疗,两组均连续治疗12周。比较两组治疗前后中医临床症状评分和关节主要指标改善情况;分析两组治疗后临床疗效;检测两组治疗前后血沉(ESR)、C-反应蛋白(CRP)和类风湿因子(RF)水平。结果:治疗组总有效率为93.18%,对照组为72.72%,治疗组显著高于对照组(χ2=5.143,P<0.05);治疗组治疗后中医临床症状评分均显著低于对照组(P<0.01);治疗组治疗后关节主要临床指标改善均优于对照组(P<0.01);治疗后,治疗组ESR、RF和CRP水平均明显低于对照组,比较差异有统计学意义(P<0.01)。结论:在西医治疗基础上,采取中药外洗方2号联合甲氨蝶呤片治疗RA寒湿痹阻型可明显降低中医临床症状评分,改善关节临床症状,降低ESR、RF和CRP水平,提高临床治疗效果。
Objective： To observe clinical efficacy of NO. 2Traditional Chinese Medicine External Wash Recipe combined with methotrexate on treating rheumatoid arthritis（ RA） with syndrome of stagnation of cold dampness. Methods： Eighty-eight RA patients of cold-dampness syndrome were randomly divided into treatment group and controlled group according to randomized digital table,44 cases for each. The controlled group were given with methotrexate tablets（ 10 mg / time,1time / week） and Meloxicam Tablets after meals（ 7. 5mg / time,bid） when severe pain. The treatment group received NO. 2Traditional Chinese Medicine External Wash Recipe（ 30 min / time,bid） on the basis of the controlled group treatment. Course of treatment of both groups：12weeks. Clinical symptom integrals and main indexes of joint were compared before and after therapy in the two groups. Treatment effects were analyzed in the two groups after treatment. Levels of erythrocyte sedimentation rate（ ESR）,c-reaction protein（ CRP） and rheumatoid factor（ RF） were detected before and after therapy in the two groups. Results： The total effective rate for the treatment group was 93. 18%,which was evidently higher than the controlled group（ χ2 = 5. 143,P〈0. 05）. After treatment,scores of TCM symptoms in the treatment group were obviously lower than the controlled group（ P〈0. 01）. After treatment,the main clinical indexes of joint of the treatment group were better than those of the controlled group（ P〈0. 01）. After treatment,ESR,CRP and RF of the treatment group were obviously lower than those of the controlled group with statistically significant differences（ P〈0. 01）. Conclusion： On the basic treatment of western medicine,NO. 2Traditional Chinese Medicine External Wash Recipe combined with methotrexate in treating RA with syndrome of stagnation of cold dampness can reduce clinical scores of TCM symptoms,improve clinical symptom of joint decrease levels of ESR,RF,and CRP,and increase clinical efficacy.
