摘要
目的对肿瘤医院静脉配置中心抗肿瘤药物不合理医嘱进行点评,探讨审核方法,以期为临床合理应用抗肿瘤药物提供参考。方法抽取2014年1月至2015年6月本院静脉配置中心抗肿瘤药物静脉用药医嘱共计19625份,审核、点评不合理医嘱情况,分析不合理医嘱的主要类型。结果经药师审核、点评共发现不合理医嘱187份,占0.95%,不合理医嘱类型主要为溶媒不合理、药物剂量不合理、给药途径不合理、给药顺序不合理、配伍禁忌等。结论针对不合理医嘱,审核药师可以加强对溶媒适宜性、给药剂量适宜性、给药途径适宜性、给药顺序适宜性等方面的审核,有效预防并纠正临床抗肿瘤药物的不合理使用以及引发的不良事件。
Objective To evaluate unreasonable doctor＇s advice of antitumor drugs in pharmacy intravenous admixture service（PIVAS） in tumor hospital. By discussing the review methods, provided a reference for clinical rational use of antitumor drugs. Methods 19625 pieces of doctor＇s advice from January 2014 to June 2015 were sampled from PIVAS in Zhejiang Cancer Hospital. Evaluated and reviewed the unreasonable advices before analyzing the major types of unreasonable advices. Results There were 187 unreasonable pieces of doctor＇s advice after pharmacists＇ examination, which accounted for 0.95%. The major types of unreasonable advices were as follows： unreasonable solvent, unreasonable drug dosage, unreasonable administration route, unreasonable administration order and compatibility taboo, etc. Conclusion According to the occurrence of unreasonable doctor＇s advice, the pharmacist can strengthen the examination in terms of the suitability of solvent, drug dosage, administration route and administration order so as to effectively prevent and correct the unreasonable clinical use of antitumor drugs and relevant adverse events.
