摘要
目的 :调查上海市2013年新发现的经异性性传播的人类免疫缺陷病毒-1型(human immunodeficiency virus-1,HIV-1)感染者病毒亚型分布情况,为探索艾滋病防治策略提供思路。方法 :对289份经异性性传播HIV-1感染者的血浆标本,应用反转录-聚合酶链反应(reverse transcription polymerase chain reaction,RT-PCR)扩增后进行DNA测序,使用HIV-Blast软件判定其亚型,并结合流行病学资料进行分析。结果 :共获得289份pol区基因片段,亚型分布显示,CRF01_AE构成比最大,为54.3%;其次为CRF07_BC,为28.7%;B亚型为7.3%;CRF55_01B为4.2%;CRF08_BC为3.5%。其中,单纯经异性性接触传播的男性感染者有168例,占男性感染者总数的65.1%(168/258),亚型分布以CRF01_AE为主,占56.0%(94/168);异性性接触合并有男男同性接触史的男性感染者有83例,占32.2%(83/258),亚型分布也以CRF01_AE为主,占61.4(51/83),略高于单纯异性接触者;异性性接触合并注射毒品途径感染者7例,占2.7%(7/258),其亚型分布以CRF07_BC为主,有6例(6/7)。结论:上海市2013年经异性性传播HIV-1感染者的亚型分布以CRF01_AE和CRF07_BC为主,但在异性性传播伴有注射吸毒史的感染者中则以CRF07_BC亚型为主。
Objective To investigate the distribution of the HIV-1 subtypes among people infected HIV-1 through heterosexual activities with no antiviral treatment in Shanghai in 2013, and explore the evidences for AIDS prevention strategies. Methods A total of 289 heterosexual HIV-l-infected patients were enrolled in the study. HIV-1 genotyping was carried out using RT-PCR, DNA sequencing, and HIV-Blast software was used to determine the subtype. The results were further analyzed with the epidemiologieal data. Results The pol gene amplicons in 289 copies revealed that CRF01_AE, CRFO7_BC, subtype B, CRF55 01B, CRFOS_BC accounted for 54.3%, 28.7%, 7.3%, 4.2% and 3.5%, respectively. 65.1%(168/258) male patients were HIV-1 infected through heterosexual contact, subtypes CRFOI_AE accounted for 56.0% (94/168). 32.2% (83/258) male patients were HIV-1 infected through both heterosexual and homosexual contact, subtypes CRFOI_AE accounted for 61.4% (51/83 ). 2.7% (7/258)male patients were HIV-1 infected through both heterosexual contact and drug injection, CRF07_BC accounted for 6 (6/7). Conclusions:CRF01_AE and CRF07_BC are the dominant of HIV-1 subtypes among those infected HIV-1 through heterosexual activities in Shanghai in 2013, however, CRF07_BC was more dominant in those HIV infections through heterosexual activities combined with injecting drug.
