摘要
目的:探讨住院精神分裂症患者便秘的患病率及影响因素,为临床患者的预防和治疗提供科学依据.方法:通过连续抽样的方法,对530例精神分裂症患者进行评估,按照Rome芋的诊断标准进行功能性便秘的诊断,应用临床总体印象评估量表(Clinical Global lmpression,CGl)和功能大体评定量表(Global Assessment of Functioning,GAF)进行疾病严重程度和患者总体功能的评定.并对导致便秘的相关因素进行单因素及Logistic多因素回归分析.结果:224例(42.2%)患者符合功能性便秘的诊断标准,Logistic回归分析显示,便秘与患者的年龄(OR:1.03,95%Cl:1.01-1.04)、女性(OR:1.60,95%Cl:1.10-2.31)、氯氮平治疗(OR:1.99,95%Cl:1.21-3.29)、高效价的-代抗精神病药物治疗(〇R:1.81,95%Cl:1.01-3.23)、蔬菜量(OR:1.25,95%Cl:1.11-4.31)、饮水量(OR:1.96,95%Cl:3.47-16.47)和活动量(OR:1.19,95%Cl:1.44-7.58)有关.结论:住院精神分裂症患者便秘患病率较高,针对高危人群应及早干预.
Objective: To investigate the prevalence and associated influencing factors of constipation in patients with schizophrenia in order to provide a scientific basis for the prevention and treatment of constipation. Methods: Five hundred and thirty patients with schizophrenia were enrolled by the successive sampling method. Rome III criteria were conducted for the diagnosis of functional constipation. The severity of the disease and the overall functions of the patients were assessed by the clinical global impression( CGI) and the global assessment of functioning( GAF),respectively. The associated risk factors of constipation were analyzed via univariate analysis and multiple Logistic regression analysis. Results: 224 patients( 42. 2%) met the criteria for functional constipation.Logistic regression analysis showed that age( OR: 1. 03,95% CI: 1. 01-1. 04),female sex( OR: 1. 60,95% CI: 1. 10-2. 31),treatment with Clozapine( OR: 1. 99,95% CI: 1. 21-3. 29),high-potency first-generation antipsychotics( OR: 1. 81,95% CI:1. 01-3. 23),vegetable and water consumption( OR: 1. 25,95% CI: 1. 11-4. 31; OR: 1. 96,95% CI: 3. 47-16. 47),and daily activities( OR: 1. 19,95% CI: 1. 44-7. 58) were the risk factors of constipation. Conclusions: A high prevalence of constipation is found in the inpatients with schizophrenia,and it should be prevented as soon as possible.
