计算机术前设计辅助椎体成形术治疗重度骨质疏松性椎体压缩骨折 被引量:4

Treatment of severe osteoporotic vertebral compression fractures with vertebroplasty using computer-assisted preoperative planning

摘要 目的探讨计算机辅助术前设计在椎体成形术治疗重度骨质疏松性椎体压缩骨折的临床应用。方法选择2007年8月~2015年3月采用计算机术前设计辅助椎体成形术治疗重度骨质疏松性椎体压缩骨折45例(46个椎体)。分别记录手术时间、手术前后患者的VAS评分,测量术前、术后骨折椎体的高度和Cobb角。结果 45例患者手术过程顺利,其中9例出现骨水泥渗漏,6例渗漏至椎间盘,3例渗漏至椎旁。术后随访2~36个月,平均(10.5±0.9)个月。手术时间15~30min,平均手术时间(21.0±3.1)min,VAS评分分别为术前(8.2±0.9)、术后(2.8±0.6)分,差异具有统计学意义(P〈0.05),椎体高度术前为(8.5±1.0)mm、术后为(11.1±1.3)mm,差异有统计学差异(P〈0.05)。术前Cobb角为(27.3±4.5)°,术后为(22.O±4.3)°,差异有统计学意义(P〈0.05)。结论计算机辅助术前设计下应用椎体成形术治疗重度骨质疏松性椎体压缩骨折,可提高手术精准性,缩短手术时间,恢复椎体高度、Cobb角,缓解疼痛,获得满意的疗效。 Objective To explore the clinical application of treatment of severe osteoporotic vertebral compression fractures with vertebroplasty using computer-assisted preoperative planning. Methods 45 severe osteoporotic vertebral compression fracture patients using computer-assisted preoperative planning were selected from August 2007 to March 2015(46 vertebral bodies).The indexes of operative time,before and after surgery in patients with VAS scores measured before surgery,postoperative vertebral height and Cobb angle were recorded. Results All the 45 cases were performed successfully,including 9 case of bone cement leakage,6 cases of leakage to intervertebral discs,3 cases of leakage to paravertebral.All cases were followed up for 2-36 months,averaged(10.5±0.9)months.Operation time was 15-30 min,averaged(21.0±3.1)min.The VAS score after operation(2.8±0.6) was significantly lower than that before operation(8.2±0.9)(P 0.05).Vertebral heights before operation were(8.5±1.0)mm,and vertebral heights after operation were(11.1±1.3)mm.There were significant differences between the two(P 0.05).Cobb angles before operation were(27.3±4.5)°,and Cobb angles after operation were(22.0±4.3)°.And the differences were statistically significant(P 0.05). Conclusion Treatment of severe osteoporotic vertebral compression fractures with vertebroplasty using computer-assisted preoperative planning could improve surgical precision,shorten operative time,restore vertebral body height and Cobb angle,relieve pain,and it could obtain satisfactory results.
