无创正压通气治疗应用于重度急性左心衰合并Ⅱ型呼吸衰竭抢救的疗效观察 被引量:4

摘要 目的探讨应用无创正压通气治疗重度急性左心衰合并Ⅱ型呼吸衰竭的临床疗效。方法选择33例重度急性左心衰合并Ⅱ型呼吸衰竭患者为观察组,选择我院同期收治的重度急性左心衰合并Ⅱ型呼吸衰竭患者32例为对照组,对照组在常规治疗基础上予持续正压通气治疗,观察组在常规治疗基础上予无创正压通气治疗,比较两组患者疗效。结果治疗后两组SBP、DBP、HR、呼吸、Pa_CO2、TEL等指标均较治疗前降低,Pa O2、PH值、LVEF、SV升高,且观察组各指标优于对照组,两组组内与组间差异均显著(P<0.05)。结论无创正压通气治疗可有效改善重度急性左心衰合并Ⅱ型呼吸衰竭患者的生命体征、缺氧症状,血气指标及心功能。 Objective To investigate the clinical effect of non-invasive positive pressure ventilation in rescue of severe acute left heart failure with type Ⅱ respiratory failure. Methods 33 cases of severe acute left heart failure with type Ⅱ respiratory failure were selected as observation group, 32 cases of severe acute left heart failure with type Ⅱ respiratory failure were selected as control group at the same time in our hospital. All patients were treated with conventional therapy. On this basis, patients in control group were treated with continuous invasive positive pressure ventilation, and patients in observation group treated with non-invasive positive pressure ventilation. The vital signs, blood gas analysis and index of heart function of two groups were compared before and after treatment. Results Compared with before treatment, the indexes of SBP, DBP, HR, breath, PaO_2, PH and TEL were decreased and Pa O2, PH, LVEF, SV were higher after treatment in two groups. Indexes of observation group were better than those of the control group. There were significant differences between intra-group and inter-group(P 0.05). Conclusion Noninvasive positive pressure ventilation in rescue of severe acute left heart failure with type Ⅱ respiratory failure can effectively improve the vital sign, hypoxic condition, blood gas index and cardiac function of patients.
