摘要
目的观察高血压左心室肥厚(LVH)人群中血浆miR-30a与LVH的关系,研究血浆miR-30a是否能作为高血压LVH患者临床诊断的标志物。方法随机入选住院高血压患者中73例LVH患者和77例非LVH患者。行real-time PCR法检测血浆hsa-miR-30a的含量。用ROC曲线和Pearson相关性分析评价血浆hsa-miR-30a诊断高血压LVH的价值。结果 LVH组患者血浆hsa-miR-30a水平升高,患者血浆hsamiR-30a水平的曲线下面积(AUC)为0.932,血浆hsa-miR-30a水平用于诊断高血压LVH有意义。确定hsamiR-30a判断高血压LVH的最佳临界点为7.267(2-Δct×104),敏感度和特异度分别为94.5%、89.6%。Pearson相关性分析表明,患者血浆hsa-miR-30a水平与室间隔厚度(IVSd)和左室后壁厚度(LVPWd)均呈正相关(R值分别为0.636和0.637,P值均为0.000)。结论高血压LVH患者血浆hsa-miR-30a升高。血浆hsamiR-30a水平与IVSd和LVPWd呈正相关,血浆hsa-miR-30a水平用于诊断高血压LVH有意义。
Objective To observe the correlation between plasma hsa-miR-30 a and left ventricular hypertrophy( LVH) in hypertension patients and to study whether plasma hsa-miR-30 a can be a diagnostic marker for patients with LVH or not. Methods Seventy-three cases with LVH were collected from the hypertensive patients as LVH group and 77 cases without LVH as the control group. The mRNA expression of hsa-miR-30 a in the derived plasma was analyzed by real-time PCR. Evaluation of the sensitivity and specificity of hsa-miR-30 a on the diagnosis of LVH was performed by analyzing the ROC curve. The association between plasma hsa-miR-30 a level and ventricular wall thickness was assessed by Pearson correlation analysis. Results Compared with that in the control group,the level of hsa-miR-30 a expression in LVH group was significantly higher [( 5. 16 ± 0. 31)( 2^- Δct× 10^4) vs( 14. 28 ± 0. 92)( 2^-Δct× 10^4),P = 0. 000]. A receiver operating characteristic( ROC) curve showed that the areas under the curve( AUC) for the plasma level of hsa-miR-30 a were 0. 932. The optimal hsa-miR-30 a cut-off value on receiver operating characteristic curve was 7. 267( 2^- Δct× 10^4). The corresponding cut-off points with sensitivity and specificity were94. 5% and 89. 6%,respectively. Pearson correlation analysis indicated that the level of hsa-miR-30 a expression in plasma was positively associated with IVSd and LVPWd)( R = 0. 636 and 0. 637,P = 0. 000 and 0. 000,respectively). Conclusion Plasma hsa-miR-30 a is elevated in hypertension patients with LVH and positively associated with left ventricular wall thickness. Elevated plasma hsa-miR-30 a level is significant for the diagnosis of LVH in hypertension patients.
