Composition analysis of urinary calculi in adult of Dongguan City

摘要 目的 分析东莞市成人尿路结石的成分组成及分布,为本地区尿石症患者病因防治提供依据。方法 收集2014年4月-2016年6月东莞地区患者送至我科检测的成人尿路结石标本(859例),包括手术治疗、体外冲击波碎石治疗及自行排石所得标本。对所有标本进行检测,并以患者年龄分组,观察不同年龄段患者尿路结石成分、分布及不同部位尿路结石成分的差异。结果 尿路结石超过90%的成分包括无水尿酸、草酸钙及草酸钙+碳酸磷灰石。其中又以草酸钙、草酸钙+碳酸磷灰石成分最多,达到39.58%、38.88%,高于其他成分(P〈0.05)。青年组中尿路结石成分以草酸钙+碳酸磷灰石为主;中年组中尿路结石成分以草酸钙为主;老年组中尿路结石成分以无水尿酸为主。尿道结石以草酸钙+碳酸磷灰石最为多见;膀胱结石以无水尿酸最多见;肾结石、输尿管结石以草酸钙、草酸钙+碳酸磷灰石较多。结论 东莞市成人尿石症主要以草酸钙、碳酸磷灰石以及无水尿酸组成,其中尿道结石、肾结石、输尿管结石以草酸钙、碳酸磷灰石为主,膀胱结石以无水尿酸为主。 Objective To analyze the component and distribution of urinary calculi for adult patients in Dongguan City and provide the basis in the prevention of urolithiasis.Methods The specimens of urinary calculi (859 cases) sent from April 2014 to June 2016 in our hospital were selected.The specimens were collected during surgery,extracorporeal shock wave lithotripsy and spontaneous passage.The subjects were divided into groups according to the patient's age. The component,distribution and position of urinary calculi were observed.Results 90% of urinary calculi were composed of ealcium oxalate,carbonate apatite and uric acid.Among them,calcium oxalate,calcium oxalate and calcium carbonate were the most,which reached 39.58% and 38.88%,which was much higher than other components (P〈0.05).For young group,the urinary calculi was majorly composed of calcium oxalate monohydrate,and carbonate apatite',for middle aged group,the urinary calculi was majorly composed of calcium oxalate monohydrate;for elderly group,the urinary calculi was majorly composed of uric acid.The urethral calculus,renal calculus and ureteral calculus were majorly composed of calcium oxalate and carbonate apatite;the vesical calculus were majorly composed of uric acid.Conclusion For adult patients in Dongguan City,the urinary calculi are majorly composed of calcium oxalate,carbonate apatite and uric acid.The urethral calculus,renal calculus and ureteral calculus are majorly composed of calcium oxalate and carbonate apatite;the vesical calculi are majorly composed of uric acid.
