摘要
目的探讨美罗培南联合奥硝唑微泵延时给药对阑尾炎术后感染高热患者相关指标的影响及临床疗效。方法选取浙江省台州市第一人民医院普外科阑尾炎术后感染高热患者52例,按随机数字表法分为2组,各26例。对照组予美罗培南治疗,研究组予美罗培南联合奥硝唑微泵延时给药治疗,观察临床症状缓解状况,治疗前后采血测定血清CRP、PCT、血清乳酸及免疫功能,同时对比临床疗效及不良反应。结果对照组治疗后有效率69.23%,低于研究组的92.31%,差异具有统计学意义(P<0.05);与对照组比较,研究组治疗后发热消退、咳嗽咳痰缓解及住院时间明显缩短,治疗后CD3+、CD4+及CD4+/CD8+、NK细胞水平增高,CD8+水平降低,治疗后CRP、PCT及血清乳酸水平降低,差异具有统计学意义(P<0.05),2组间不良反应比较差异无统计学意义。结论采用美罗培南联合奥硝唑微泵延时给药显著降低阑尾炎术后感染高热患者血清CRP、PCT水平,提高免疫功能,疗效显著。
Objective To explore the effect of meropenem combined with ornidazole minipump delay drugs delivery on related indicators and T in appendieitis of postoperative infection and high fever and its efficaey. Methods A total of 52 patients with appendicitis of postoperative infection and high fever from department of general surgery of Taizhou first people ' s hospital were selected and randomly -divided into two groups with 26 cases in each group. Patients in the eontrel group were treated by meropenem, patients in the experimental group were treated by meropenem eombined with ornidazole minipump delay drugs delivery. The clinical symptoms remission, the serum CRP, PCT, laetie acid and immune function before and after treatment were observed, and the elinieal efficacy and adverse reaetions were compared. Results After treatment, the effeetive rate of the control group was 69.23%, which was lower than 92. 31% of the experimental group (P 〈 0. 05 ) ; compared with the control group, the remission of fever, eough and expectoration, length of hospital stay of the experimental group significantly decreased ( P 〈 0. 05 ) ; after treatment, CD3^+ , CD4^+, CD4^+/CD8^+ and NK eells levels increased, CD8^+ level decreased, serum CRP, PCT and lactic acid levels decreased after treatment (all P 〈 0. 05 ), there was no adverse reactions between two groups. Conclusion Meropenem combined with oruidazole minipump delay drugs delivery in the treatment of patients with appendicitis of postoperative infection and high fever could reduce the serum CRP and PCT levels, improve immune funetion, whose curative effect is remarkable.
