高通量血液透析缓解维持性血液透析患者微炎症状态与营养改善状态的研究 被引量:13

摘要 目的探讨高通量血液透析缓解维持性血液透析患者微炎症状态与营养改善状态。方法选择2013年10月至2014年10月四川省绵阳市四O四医院收治的维持性血液透析患者152例,按照随机数字表法分为观察组与对照组,各76例。观察组患者采取高通量[膜超滤系数Kuf20.0~55.0mE/(h·mmHg),尿素清除率3.08~3.20mL/s]血液透析,对照组采用低通量[膜超滤系数Kuf4.2~8.0mL/(h·mmHg),尿素清除率3.00~3.17mL/s]血液透析。观察两组患者治疗后微炎性因子水平下降情况[白细胞介素6(IL-6)、肿瘤坏死因子α(TNF—α)、高敏C反应蛋白(hs—CRP)]及营养指标增长情况[总蛋白(TP)、体质指数(BMI)、白蛋白(ALB)],营养评分量表(SGA)评分。结果治疗后,观察组IL-6、TNF-α、Hs—CRP水平显著低于对照组[(149.7±21.3)ng/L比(310.4±41.4)ng:,/L,(2.2±0.5)μg/L比(5.4±1.0)μg/L,(2.6±0.5)mg/L比(5.5±1.2)mg/L](P〈0.01);TP、ALB显著高于对照组[(72.3±4.3)g/L比(61.5±4.2)g/L,(38.7±1.6)g/L比(32.4±1.5)g/L],BMI显著高于对照组[(29.2±2.2)kg/m^2。比(22.0±2.0)kg/m^2](P〈0.01);观察组营养改善状态优于对照组(P〈0.05)。结论患者长时间的高通量血液透析后微炎症状态和营养状态均有明显的改善,是维持性血液透析患者的血液透析选择较佳的方式。 Objective To study the high-throughput hemodialysis improvement on microinflammation and nutrition status in maintenance hemodialysis patients. Methods Total of 152 cases of maintenance hemodialysis patients in Sichuan Mianyang 404 Hospital from Oct. 2013 to Oct. 2014 were included in the study, according to the random number table method they were divided into an observation group and a control group ,76 cases in each group. The observation group was given high-throughput hemodialysis [ ultrafihration coefficient Kuf 20. 0-55.0 mL/( h · mmHg) ,urea clearance 3.08-3.20 ml./s] ,the control group was giv- en low-throughput hemodialysis [ ultrafiltration coefficient Kuf 4. 2-8.0 mL/( h · mmHg), urea clearance 3.00- 3. 17 mL/s ], the microinflammatory eytokine levels deereaseafter treatment [ interleukin-6 ( IL-6 ), tumor nec- rosis factor ct(TNF-ct) ,high-sensitivity C-reactive protein(hs-CRP) ] and nutrition indicators growth [total protein (TP),body mass index (BMI), albumin (ALB)], nutrition rating scale (SGA) score of the two groups after treatment were observed. Results After treatment, the IL-6, TNF-α, Hs-CRP levels of the observation group were significantly lower than the control group [ ( 149. 7 ± 21.3 ) ng/L vs (310. 4 ±41.4) ng/L, (2.2±0.5) μg/L vs (5.4±1.0) μg/L,(2.6 ±0.5) mg/L vs (5.5 ±1.2) mg/L] (P〈0.01);TP,ALB of the observation group was significantly higher than the control group [ ( 72. 3 ± 4. 3 ) g/L vs ( 61.5 ± 4. 2 ) g/L, (38.7 ± 1.6 ) g/L vs ( 32. 4 ± 1.5 ) g/L ], BMI was significantly higher than the control group [ (29. 2 ± 2. 2 ) kg/m2 vs (22.0 ± 2. 0 ) kg/m2 ] ( P 〈 0. 01 ). The nutritional status improvement of the observation group was better than the control group ( P 〈 0. 05 ) Conclusion The microinflammation and nutritional status are significantly improved after a long period of high-throughput hemodialysis, which is a better way for the maintenance hemodialysis.
