摘要
目的探讨血清Vaspin在2型糖尿病及合并不同时期视网膜病变患者中的变化及其临床意义。方法根据眼底荧光造影,将126例2型糖尿病患者分为单纯糖尿病组(NDR组)43例、糖尿病合并背景期视网膜病变组(SDR组)41例、糖尿病合并增殖期视网膜病变且未行相关治疗组(PDR组)42例。同时选择健康体检者40例为正常对照组(NC组),测定各组血清Vaspin及C反应蛋白(CRP)、空腹血糖(FPG)、餐后2小时血糖(2hPG)、糖化血红蛋白(HbA1c)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDLC)、总胆固醇(TC)、甘油三酯(TG)。比较血清Vaspin在正常对照组、单纯糖尿病组及不同时期糖尿病视网膜病变组的变化,并采用Pearson相关分析探讨血清Vaspin与相关指标的关系。结果 1血清Vaspin水平依次为:NC组<PDR组<SDR组<NDR组。2NDR组血清Vaspin水平与CRP、FPG、HbA1c、LDL-C、HDLC有显著相关性(r分别为0.539、0.371、0.342、0.501、-0.414,P<0.01或P<0.05)。糖尿病合并视网膜病变组(包括SDR组和PDR组),血清Vaspin水平与CRP、LDL有显著相关性(r分别为-0.423、-0.341,P<0.01或P<0.05)。结论血清Vaspin水平在初发2型糖尿病人群中升高,随着视网膜病变的发生、发展逐渐下降。Vaspin对糖尿病微血管病变起重要作用。
Objective To investigate the change of serum Vaspin in type 2 diatbtic retinopathy during different stages as well as its clinical significance. Method According to the fundus fluorescein angiography,a total of 126 patients with type 2 diabetes were divided into no retinopathy（ NDR）,background DR group（ SDR） and proliferation of DR group（ PDR）,40 healthy volunteers were also enrolled as control group（ NC）. The Vaspin and fasting plasma glucose（ FPG）,2h postprandial plasma glucose（ 2h PG）,glycosylated hemoglobin（ HbA1c）,low density lipoprotein cholesterol（ LDL）,high density lipoprotein cholesterol（ HDL）,total cholesterol（ TC）,triglycerides（ TG） and C-reactive protein（ CRP） were measured. The blood Vaspin level was compared among these group. The relationship of Vaspin level with biochemical indices were analysed using Preason correlation. Result ① The serum Vaspin level of four groups were in order of NC group 〈PDR group〈 SDR group 〈NDR group. ②In the NDR group,the Vaspin level were significantly correlated with FPG,HbA1c,LDL,HDL and hs-CRP level（ P〈 0. 01 or P〈0. 05）. In the diabetic retinopathy group（ including SDR group and PDR group）,the Vaspin level were significantly correlated with CRP and LDL（ P〈 0. 01 or P〈0. 05）. Conclusion Serum Vaspin is significantly elevated in type 2 diabetic patients and its level is decreased gradually with the occurrence and development of retinopathy. Vaspin play a very important role in type 2 diabetes mellitus with microvascular complications.
