摘要
目的降低择期手术患者术前准备不足发生率,提高术前准备质量。方法组建非手术室护士专职交接团队,进入病区对择期手术病人进行术前床边交接,前移术前交接管理关口,记录交接过程中发现的术前准备不足的发生类别、发生频次。统计专职术前床边交接初期与实施1年后术前准备不足数据,将专职术前床边交接初期(2013年12月至2014年2月)3339例择期手术患者术前准备情况设为对照组,实施1年后(2014年12月至2015年2月)4111例择期手术患者的术前准备情况设为观察组,比较两组术前准备不足发生率和构成比。结果术前准备不足发生率从4.34%下降至0.39%,手术部位、患者身份、术前带药、患者准备、术前检验等五项术前准备不足发生率差异均有统计学意义(P<0.05);同时在手术准备不足构成比发生明显变化,为管理督导提供了方向。结论护士专职术前床边交接,使术前交接管理"关口前移"成为可能,使各项术前准备能够在合适的地点与时间内完成,可降低择期手术患者术前准备不足的发生率,提高术前准备质量。
Objective To reduce inadequate preoperative preparation for selective surgery patients and to improve the quality of preoperative preparation. Methods A professional team of non-operation room nurses was established to enter the ward in order to conduct bedside handover for selective surgery patients, advance the management of preoperative handover and record the category and frequency of inadequate preoperative preparation. Data of inadequate preoperative preparation at the beginning period of implementing preoperative handover and one year after the implementation was analyzed. In total of 3339 selective surgery patients at the beginning period of implementing preoperative handover(from December 2013 to February 2014) were included in control group and 4111 selective surgery patients one year after the implementation(from December 2014 to February 2015) were included in observation group. Incidence rates and constituent rates of inadequate preoperative preparation in the two groups were compared. Results The rate of insufficient preoperative preparation decreased from 4.34% to 0.39%. There were statistically significant differences of incidences rates of five categories of insufficient preoperative preparation including surgical site, patient identification, preoperative preparation of medication, patients with preoperative preparation and preoperative examination(P0.05); at the same time, there were significant changes of constituent rates of insufficient preoperative preparation, which provided the direction for the management and supervision. Conclusions Preoperative bedside handover conducted by professional nurses makes the advancement of preoperative handover management possible and makes the preoperative preparation be completed in a suitable location and at appropriate time in order to reduce the incidence rate of insufficient preoperative preparation of selective surgery patients and improve the quality of preoperative preparation.
