摘要
目的探讨经睫状体平坦部应用针头固定脱位晶状体行超声乳化吸出术对晶状体半脱位的手术效果。方法对2009年1月至2014年12月在无锡人民医院眼科的晶状体核硬度〈HI级、脱位范围在1-3个象限的晶状体半脱位者48例(48只眼),经睫状体平坦部应用针头固定脱位晶状体行超声乳化吸出术治疗后,观察术后视力、IOL的位置和手术并发症。结果视力较术前不同程度提高45例(93.75%)。所有患者IOL位置稳定,47只眼IOL襻均位于睫状沟内,并与虹膜后保持一定的距离,对虹膜无摩擦和损伤,1只眼的上襻扭曲致部分虹膜后被牵拉。18只眼术后发生角膜轻度水肿,3只眼前房内残留少许黏弹剂,致术后眼压增高,2天后恢复正常。2只眼瞳孔轻度上移;瞳孔变形3只眼。结论经睫状体平坦部应用针头固定脱位晶状体行超声乳化吸出术是一种简单有效地治疗晶状体半脱位的方法。
Objective To evaluate surgical effect for the treatment of subluxation lens by phacoemulsification surgery by inserting a needle into the lens via pars plana. Methods Lens nucleus had a grade Ⅰ to Ⅲ nucleus hardness (according to LOCS grade), dislocation range was from 1 to 3 quadrants lens subluxation in 48 cases (48 eyes). The phacoemulsification surgery for lens dislocation was done with inserting a needle into the lens via pars plana. Postoperative visual acuity, the position of IOL and surgical complications were observed, Results Visual acuity was improved at different levels in 45 cases (93.75%). The position of IOL was stable in all patients, with IOL haptics located in the ciliary sulcus in 47 eyes, and maintained a certain distance from the iris, and the iris had no friction and damage. The iris was tract in only one eye by the distorted superior loop of IOL. Corneal edema in 18 eyes, pupil upward displacement mildly in 2 eyes and pupil deformation in 3 eyes were observed postoperatively. A little residual viscoelastic in 3 anterior chamber induced high intraocular pressure which rerumed to normal two days later. Conclusions Application of phacoemulsification surgery by inserting a needle into the lens via pars plana is a simple and effective method for the treatment of lens subluxation.
