摘要
目的探讨达芬奇机器人系统应用于胃癌全胃切除脾门淋巴结清扫中的可行性及安全性。方法回顾性分析四川大学华西医院胃肠外科中心于2016年1月进行的两例因贲门癌而行全胃切除患者的临床资料。结果两例患者分别为62岁和55岁男性,术前均通过胃镜及活检明确诊断为cT2~3N0M0期和cT1~2N0M0期胃癌。均成功接受了机器人全胃切除保留脾脏的脾门淋巴结清扫,清扫时间分别为30 min和25 min。术中出血量均为100 ml,手术清扫淋巴结数目分别为38枚和33枚,所清扫的脾门淋巴结分别为1枚和纤维脂肪组织。未出现手术相关并发症以及吻合口瘘、胰漏、脾梗死、腹腔出血、消化道出血、脾动脉瘤等并发症。术后两例患者均并发肺部感染,经保守治疗痊愈后顺利出院。结论机器人系统对于全胃切除保留脾脏的脾门淋巴结清扫安全、可行,但其优势还有待进一步评估。
Objective To investigate the feasibility and safety of Da Vinci surgical robot in the dissection of splenic bilar lymph nodes for gastric cancer patients with total gastreetomy. Methods Clinical data of two cases who underwent total gastrectomy for cardia cancer at our department in January 2016 were analyzed retrospectively. Results Two male patients were 62 and 55 years old respectively, with preoperative diagnosis as cT2-3N0M0 and cT1-2N0M0 gastric cancer by gastroseope and biopsy, and both received robotic total gastrectomy spleen-preserving splenic hilar lymph node dissection successfully. The operative time for splenic hilar lymph node dissection was 30 min and 25 min respectively. The intraoperative estimated blood loss was both 100 ml, while the number of total harvested lymph node was 38 and 33 respectively. One dissected splenic hilar lymph node and fatty tissues in two patients were proven by pathological examinations. There were no anastomotic leakage, pancreatic fistula, splenic infarction, intraluminal bleeding, digestive tract bleeding, aneurysm of splenic artery, and other operation-associated complications. Both patients suffered from postoperative pneumonia, and were cured by conservative therapy. Conclusion The robotic spleen-preserving splenic hilar lymph node dissection is feasible and safe, but its superiority needs further evaluation.
