ICU气管切开患者下呼吸道感染病原菌与相关因素分析 被引量:15

摘要 目的探讨ICU气管切开患者伴发下呼吸道感染病原菌与相关影响因素,为ICU感染预防控制提供参考依据。方法选取医院2013年5月-2014年5月收治的382例ICU气管切开患者为研究对象,分析感染率,并对感染患者分泌物进行病原菌培养并对医院感染的常见病原菌进行分析,分析其感染因素,数据采用SPSS 18.0软件进行统计分析。结果在382例ICU气管切开患者中,共有153例患者发生下呼吸道感染,感染率为40.05%;共检出172株病原菌,以革兰阴性菌为主,共125株占72.67%;多因素logistic回归分析结果显示:住ICU时间、呼吸衰竭、肺气肿、侵入性操作、使用抗菌药物种类数、年龄、气道开放时间、雾气吸入是CU气管切开患者伴发下呼吸道感染的独立因素(P<0.05)。结论应针对易感染病原菌和危险因素制定相对应的措施,加强预防与控制患者下呼吸道感染的发生,降低该病的发生率。 OBJECTIVE To explore the characteristics of pathogenic bacteria and related factors affecting lower respiratory tract infection of patients after tracheotomy in ICU,so as to provide evidence for the prevention and control of infections in ICU.METHODS A total of 382 cases of patients with tracheotomy in ICU of our hospital from May 2013 to May 2014 were selected as the research subjects.The infection rate was analyzed,and the pathogens were cultured and the common pathogens of nosocomial infections were analyzed.The single factor analysis and multi-factors logistic regression analysis were performed on the clinical data of the patients.All data were statistically analyzed by SPSS 18.0.RESULTS There were 153 cases were found to have lower respiratory tract infections in the 382 cases of patients with tracheotomy in ICU,and the infection rate was 40.05%.A total of 172 strains of pathogens were detected,among which the gram-positive bacteria accounted for 23.26%,gram-negative bacteria for 72.67% and fungi for 4.07%.The multiple factors logistic regression analysis showed that ICU stay time,respiratory failure,emphysema,invasive operations,types of used antibiotics,age,airway opening time,mist inhalation were related independent risk factors for lower respiratory tract infections of patients after tracheotomy in ICU(P〈0.05).CONCLUSION Measures should be taken according to pathogens detected and risk factors so as to prevents and control lower respiratory tract infection and reduce the infection rate.
