摘要
目的:探究用自拟消痹汤内服联合外敷治疗类风湿性关节炎的有效性。方法:将2011年7月—2015年7月就诊于我院的60例类风湿关节炎患者随机分为对照组和观察组患者各30例。对照组患者给予甲氨蝶呤片内服,观察组患者则联合给予自拟消痹汤内服联合外敷治疗,连续治疗3个月。治疗前后记录关节晨僵持续时间、肿胀关节数、疼痛关节数,用疼痛模拟视觉VAS评分评估痛感,监测类风湿因子(RF)、血沉(ESR)、炎性因子、免疫力水平变化,评价临床治疗的有效性。结果:观察组患者临床症状改善优于对照组患者,关节晨僵持续时间明显缩短,肿胀、疼痛关节数显著减少,关节疼痛评分显著低于对照组,有效率93.3%显著高于对照组有效率70%,差异性显著(P<0.05);观察组患者RF、ESR、炎性因子CRP、IL-6、IL-β水平均较治疗前及对照组降低,差异性显著(P<0.05);观察组患者的机体免疫力显著提高,其T细胞亚群CD4+、CD4+/CD8+,血清免疫球蛋白Ig G、Ig A、Ig M水平显著高于对照组患者,而CD8+水平则明显低于对照组患者,差异性显著(P<0.05)。结论:自拟消痹汤内服联合外敷治疗类风湿性关节炎疗效确切,利于改善患者的临床症状、改善患者的生化指标,其机制与降低炎性因子水平、增强机体免疫力有一定相关性。
Objective：To investigate different clinical efficacy of treating rheumatoid arthritis patients with Self-made Xiaobi Decoction for internal and external application. Methods：60 patients with rheumatoid arthritis were randomly divided into a control group of 30 cases and an observation group of 30 cases from July 2011 to July 2015. Patients in the control group were treated with methotrexate tablets for internal application;while patients in the observation group were treated with Self-made Xiaobi Decoction for internal and external application,3 months for treatment. The research was aimed at recording joint morning stiffness duration,the number of pain joint and swelling joints and the pain change of joints,monitoring the changes of RF,ESR,inflammatory factor and immunity,and evaluating the efficacy of the treatment. Results：The improvement of main clinical symptoms of the patients in the observation group was better than that of the patients in the control group,the joint morning stiffness duration was significantly shorten compared with the control group,the number of pain joint and swelling joints were less than that of the control group,and the pain scores of joints were lower than that of the control group,and the efficiency was 93.3%,which was higher than that of the control group 70%,the difference was significant（P〈0.05）;The levels of RF,ESR,hs-CRP,IL-6 and IL-β were lower than that of the control group patients,the difference was significant（P〈0.05）;while the patients＇ immunity of the observation group was significantly improved,the levels of CD4＋,CD4＋/CD8＋,Ig G,Ig A,Ig M were higher than that of the control group patients,while the level of CD8＋ was lower than that of the control group patients,the difference was significant（P〈0.05）. Conclusion：The therapy has good hypoglycemic effect,it can improve the patients＇ clinical symptoms and biochemical index,the mechanism may be associated with lowing the levels of inflammatory cytokines and enhancing immunity.
