摘要
目的:探讨他克莫司对狼疮性肾炎血清胱抑素C、血肌酐及尿素氮的影响。方法:狼疮性肾炎患者74例随机分为观察组与对照组,每组37例。观察组予他克莫司治疗,对照组予环磷酰胺治疗,疗程均为1年。比较两组患者治疗前后的24h尿蛋白定量、血清白蛋白、血红蛋白、补体C3、系统性红斑狼疮疾病活动度评分(SLEDAI)、血清胱抑素C、血肌酐及尿素氮等指标变化,观察两组疗效和药品不良反应。结果:治疗1年后,观察组尿蛋白定量、SLEDAI、血清胱抑素C、血肌酐及尿素氮水平等指标低于对照组,血清白蛋白、血红蛋白、补体C3水平高于对照组,差异均有统计学意义(P<0.05)。观察组总有效率为91.89%,明显高于对照组(P<0.05),且观察组药品不良反应发生率明显比对照组低(P<0.05)。结论:他克莫司治疗狼疮性肾炎可有效降低血清胱抑素C、血肌酐及尿素氮水平,安全性较好。
Objective: To study the effect of tacrolimus on cystatin C,serum creatinine and urea nitrogen in the patients with lupus nephritis. Methods: Totally 74 cases of patients with lupus nephritis were divided into the observation group and the control group with37 cases in each according to the random number table. The observation group was treated with tacrolimus,the control group was treated with cyclophosphamide,treatment course was 1 year. Before the treatment and after the one-year treatment,the 24 h urine protein,serum albumin,hemoglobin,quantitative C3 disease activity,systemic lupus erythematosus score( SLEDAI),cystatin C,serum creatinine and urea nitrogen in the two groups were compared,and the clinical efficacy and adverse reactions were observed. Results: After the one-year treatment,the urine protein quantitation,SLEDAI,cystatin C,serum creatinine and urea nitrogen in the observation group were lower than those in the control group,and the serum albumin,hemoglobin and complement C3 levels were higher than those in the control group with significant differences( P 〈0. 05). The total clinical efficacy rate of the observation group was significantly higher than that of the control group( P 〈0. 05). The adverse reaction rate of the observation group was obviously lower than that of the control group( P 〈0. 05). Conclusion: Tacrolimus used in the patients with lupus nephritis can effectively reduce cystatin C,serum creatinine and urea nitrogen levels,which is one of safe and effective drugs.
