摘要
目的分析右美托咪定对高血压患者全身麻醉拔管期RPP(心率收缩压乘积)和PRQ(血压心率比值)的影响。方法选取全身麻醉下进行手术的高血压患者115例,随机分为治疗组60例和对照组55例,在拔管期对照组给予常规的生理盐水注射,治疗组给予右美托咪定注射。分析两组患者在拔管期间以及拔管过后5 min、10 min的SBP、MAP、HR、DBP的变化,从而得出对RPP和PRQ的影响。结果两组患者SBP、MAP、HR、DBP值在拔管期间都有所升高,治疗组升高的幅度比对照组低,对比差异有统计学意义(P<0.05);两组患者SBP、MAP、HR、DBP值在拔管后5 min以及拔管后10 min均有降低,而治疗组降低较明显,对比差异有统计学意义(P<0.05)。结论在高血压患者全身麻醉拔管前30 min,给予右美托咪定静脉滴注,可有效稳定患者的血流动力学和心血管反应,从而降低RPP,应用价值较高。
Objective To analyze the effect of dexmedetomidine on hypertension patients with general anesthesia extubation RPP(ratepressure product) and PRQ(the ratio of blood pressure and heart rate) effect. Methods 115 patients with hypertension underwent surgery under general anesthesia were randomly divided into treatment group 60 cases and control group 55 cases, pull on tube control group were given normal saline injection routine, treatment group were given dexmedetomidine injection. Analysis of patients in the two groups during extubation and extubation after 5 minutes, 10 minutes of SBP, MAP, HR, DBP changes, so that the influence on RPP and PRQ. Results Two groups of patients with SBP, MAP, HR, DBP value during extubation increased, the treatment group was significantly lower than the control group, there were statistically significant differences between two groups of patients(P0.05), SBP, MAP, HR, DBP value at 5 min after extubation and 10 minutes after extubation were decreased, while the treatment group decreased obviously, the difference compared with statistical significance(P0.05). Conclusion In hypertensive patients with general anesthesia pull 30 min before the tube, given dexmedetomidine infusion, hemodynamic and cardiovascular responses in patients with stable and effective, thereby reducing the RPP and higher application value.
