右美托咪定对心肺转流心脏瓣膜置换术患者肾损伤的影响 被引量:2

摘要 目的观察右美托咪定对心肺转流(CPB)心脏瓣膜置换患者肾损伤的影响。方法选择择期CPB下行二尖瓣置换术的风湿性心脏病患者40例,美国麻醉医师协会(ASA)分级为Ⅱ或Ⅲ级,分为健康对照组(C组)和右美托咪定组(D组)各20例。D组于麻醉诱导前10min静脉缓慢泵注右美托咪定1μg/kg,继之以0.5μg·kg^(-1)·h^(-1)速率持续泵注至术毕,C组以等量生理盐水代替,两组麻醉诱导及维持方法相同。监测两组患者术中一般情况,分别于麻醉诱导前(T0)和CPB开始(T_1)、CPB后6h(T_2)、12h(T_3)、24h(T_4)、48h(T_5)和72h(T_6)时点记录血流动力学变化和采集动脉血测定中性粒细胞明胶酶脂质运载蛋白(NGAL)和胱抑素C(CysC)的浓度。结果两组患者术中一般情况和血流动力学差异无统计学意义。与T_0相比,两组T_2、T_3、T_4时NGAL和CysC浓度明显升高(P<0.05)。与C组相比,D组T_2、T_3、T_4时NGAL和CysC浓度明显降低(P<0.05)。结论 CPB下行二尖瓣置换术患者的NGAL和CysC浓度升高,而右美托咪定可降低其浓度,减轻肾损伤。
