超声心动图对肺栓塞高危组右心功能及治疗效果的评估 被引量:3

VALUE OF ULTRASOUND CARDIOGRAM IN ASSESSING THE RIGHT HEART FUNCTION AND TREATMENT EFFECT FOR SERVER PULMONARY EMBOLISM PATIENTs

摘要 目的通过超声心动图技术的联合应用探讨肺栓塞高危组右心功能的超声心动图分型、分析,评估治疗的效果。方法选取同期住院的肺栓塞高危组患者76例,均接受尿激酶溶栓治疗。治疗前和治疗2周后均观察超声心动图检查指标,包括实时三维超声心动图(RT-3DE)技术测量右心室舒张末期容积(RVEDV)及射血分数(RVEF);多普勒超声测量三尖瓣反流压力阶差(TRPG)及三尖瓣口前向血流舒张早期峰值速度(Em);组织多普勒成像技术(TDI)测量三尖瓣环心肌运动舒张早期峰值速度(E'm),计算Em/E'm,观察比较指标变化。结果治疗后超声心动图显示,患者的RVEDV、TRPG、Em/E'm较治疗前显著减小,RVEF较治疗前显著提高,差异有统计学意义(P<0.05)。结论通过超声心动图技术的联合应用对肺栓塞高危组右心功能进行分型、评估,可帮助临床医生了解其预后效果。 Objective To discuss the function of right ventricular in the patients with the server pulmonary .embolism in ultrasound cardiogram types combined application of echocardiography, and to evaluate its therapeutic effect. Methods Seventy- six patients with the server pulmonary embolism were selected who accepted urokinase thrombolytic therapy. Echocardiography was performed before treatment and two weeks after treatment, and ultrasonic cardiography graph examination indexes were observed. It included real time three- dimensional echocardiography( RT - 3DE) technique measurement of patients of right ventricular end diastolic volume(RVEDV) and ejection fraction(RVEF) ; Doppler ultrasound measurement of three tri- cuspid regurgitation tricuspid valve differential pressure(TRPG) and three of early diastolic peak velocity (Era) of blood flow. Measurement of tissue Doppler imaging of three tricuspid annulus peak early diastolic velocity (E'm) of myocardial motion. Then the Era/ Era was calculated. Results After urokinase throm- bolytic therapy, RVEDV,TRPG and Era/ Era were decreased obviously, but RVEF was raised obviously and the differences were significant(P〈0.05). Conclusion According to the ultrasound cardiogram of the right ventricular function of pulmonary embolism analysis, the typing can help clinicians to understand its prognosis effect.
