摘要
本研究将78例早发冠心病急性心肌梗死患者采用随机数表分为A组(39例)和B组(39例),所有患者均给予基础治疗,A组另给予瑞舒伐他汀,B组另给予阿托伐他汀。对比两种他汀类药物治疗前后患者血管内皮功能、心脏功能相关指标水平变化,并观察血脂和hs-CRP水平变化。随访一年,比较不良心血管事件发生率。结果发现在治疗前和治疗后两组FMD、LVEF、IMT水平比较均无显著差异(p>0.05),且治疗后两组各项水平均较治疗前显著改善(p<0.05);治疗前两组TG、TC、HDL-C、LDL-C和hs-CRP水平无显著差异(p>0.05),治疗后除HDL-C无明显改善外,其他因子水平均较治疗前显著改善(p<0.05),且治疗后A组TC、LDL-C和hs-CRP水平均显著优于B组(p<0.05);A组不良心血管事件发生率为7.69%,B组为12.82%,前者稍低于后者,但差异无统计学意义(p>0.05)。提示瑞舒伐他汀和阿托伐他汀在改善早发冠心病急性心肌梗死患者血管内皮功能和心脏功能方面作用相近,但前者降脂和减轻炎症反应效果更为显著,不良心血管事件少。
In this study, 78 cases of early onset coronary artery disease in patients with acute myocardial infarction using stochastic indicator was divided into group A (39 cases) and group B (39 cases). All patients were given basic treatment, the other group A was given them comfortable rosuvastatin and given atorvastatin group B another. Compared to two kinds of vascular endothelial function in patients with statins before and after treatment, vascular endothelial function, level change of the related parameters in cardiac function, blood fat and change of the hs-CRP levels were observed. Follow-up of 1 year, the incidence of adverse cardiovascular events were compared. We found the two groups before and after treatment, the levels of FMD, LVEF, IMT were no significant differences (p〉0.05). From all levels in the two groups, compared before treatment, after treatment were significa- ntly improved (P〈0.05). Two groups before treatment, the levels ofTG, TC, HDL-C, LDL-C and hs-CRP had no si- gnificant difference (P〉0.05). In addition to the HDL-C level, there was no obvious improvement and the other fa- ctor levels were significantly improved (P 〈0.05) after treatment. Furthemore, the levels of TC, LDL-C and hs-CRP were significantly better than that of group B (P〈0.05). Incidence of adverse cardiovascular events was 7.69% in Group A, and group B was 12.82%. The former was slightly lower than the latter, but there was no statistically significant difference (P 〉0.05). Rosuvastatin and atorvastatin had similar effects in improving vascular endothelial function and heart function of patients in early onset coronary artery disease with acute myocardial infarction, but the former had significant effect of lipid-lowering and reduce inflammation reaction, as well as had less adverse cardiovascular events.
