糖肾煎对早期2型糖尿病肾病大鼠足细胞α-actinin-4、ZO-1表达的影响 被引量:8

摘要 目的:探讨α-辅肌动蛋白-4(α-actinin-4)、紧密连接蛋白(ZO-1)在早期糖尿病肾病(DN)大鼠肾脏表达与DN的关系,中药糖肾煎对保护肾脏的作用机制。方法:采用单肾切除+高脂饲料+小剂量链脲佐菌素(STZ)腹腔注射,结合行为干预建立早期(气阴两虚型)2型糖尿病肾病病证结合大鼠模型,按随机数字表法分为假手术组(N组)、模型组(M组)、糖肾煎组(T组)、贝那普利组(B组)。干预16周,观察各组大鼠一般状况、空腹血糖(FBG)、负荷后血糖(P2h BG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、24 h尿白蛋白(24 h-UAlb)、尿β2微球蛋白(U-β2MG)、血尿素氮(BUN)、血肌酐(Cr))、足细胞密度及病理改变,应用免疫组化及荧光定量PCR方法观察α-actinin-4、ZO-1及mRNA表达。结果:与N组比较,M组一般状况较差,FBG、P2h BG、FINS、HOMA-IR、24 h-UAlb、U-β2MG水平指标均有明显上升,α-actinin-4、ZO-1的累积光密度值(IOD)下降,PCR表达下降,足细胞密度较少,足突增宽、融合;与M组比较,T、Z组各指标均有改善;与B组比较,T组HOMA-IR、α-actinin-4、ZO-1的IOD有所改善,余指标比较差异无统计学意义。结论:中药糖肾煎能上调足细胞α-actinin-4、ZO-1的表达从而保护足细胞、延缓DN的进展。 Objective: To investigate the α-actinin-4、ZO-1 in early diabetic nephropathy( DN) in the rat kidney expression and traditional Chinese medicine Tangshenjian on its interventi-on,to study the effect of the renal protective mechanism. Methods: using single renal resection + high-fat + small dose of streptozocin( STZ) intraperitoneal injection,combined with behav-ior intervention,establish early( Qiyin deficiency type) in type 2 diabetic nephropathy ratsmodel,were rando-mly divided into control group( N group),model group( M),Tangshenjiangroup( group T),the Captopril group( group B). Intervention to observe each group for 16 weeks general condition,blood and urine of rats biochemical indicators( fasting blood glucose( FBG),load after blood glucose( P2 h BG),fasting insulin( FINS),insulin resistance index( HOMA IR),24 Hurinary albumin( 24 h-UAlb),urine β2 microglobulin( U-β2MG),blood ureanitrogen( BUN),serum creatinine( Cr),the density of podocyte and pathological changes of thepodocyte undere lectron microscope,immunohistochemical and fluorescence quantitative PCRmethod is appl-ied to observe Alpha auxiliary actin- 4( α-actinin-4) 、zonula occludens-1( ZO-1) proteinand mRNA expression changes. Results: compared with N group,group Mpoor generalcondition,FBG,P2 h BG,FINS,HOMA IR,24 h-UAlb,U-β2MG level indicators aresignificantlyincreased,alpha actinin-4、ZO-1proteinthe cumulative values optical density( IOD),PCR expression podocytedensity decreased,foot process broadening andfusion; compared with group M,T,each index Z group were improved; compared with group B,group T HOMA IR,α-actinin-4,ZO-1 IOD improved,the index has no obvious difference. Conclusion: Tangshenjian can improvethe early DN rat general condition and related indicators,such as urinary protein,it may be through the podocyte α-actinin-4、ZO-1 expression so as to protect the podocyte cell,delay the progress of DN.
