摘要
目的探讨前列地尔对急诊经皮冠状动脉介入治疗(PCI)术后急性ST段抬高型心肌梗死患者室性心律失常的影响。方法选取2015年1月—2016年3月聊城市第二人民医院心内科收治的行急诊PCI的急性ST段抬高型心肌梗死患者100例,按照随机数字表法分为对照组和试验组,每组50例。两组患者入院后均进行常规处理并行急诊PCI,观察组患者于PCI术前30 min给予前列地尔治疗。比较两组患者室性心律失常发生情况、QT间期离散度及校正的QT间期离散度(QTcd)。结果试验组患者室性期前收缩发生率及非持续性室性心动过速+心室颤动发生率低于对照组(P<0.05)。时间与方法存在交互作用(P<0.05);时间在QT间期离散度、QTcd上主效应显著(P<0.05);方法在QT间期离散度、QTcd上主效应显著(P<0.05);试验组患者术后6、24、48、72 h QT间期离散度、QTcd均低于对照组(P<0.05)。结论前列地尔对行急诊PCI术的急性ST段抬高型心肌梗死患者有一定的心肌保护作用,可有效降低患者PCI术后室性心律失常发生率,这可能与前列地尔抗炎、抗氧化及改善微循环等作用有关。
Objective To investigate the impact of alprostadil on ventricular arrhythmias in postoperative ASTEMI patients treated by emergency PCI. Methods From January 2015 to March 2016,a total of 100 patients with ASTEMI were selected in the Department of Cardiology,the Second People′s Hospital of Liaocheng,all of them were treated by emergency PCI,and they were divided into control group and experiment group according to random number table,each of 50 cases. Patients of both groups received conventional treatment and emergency PCI,meanwhile patients of experiment group received alprostadil 30 minutes before PCI. Incidence of ventricular arrhythmias,QT interval dispersion and corrected QT interval dispersion(QTcd) were compared between the two groups. Results The incidence of ventricular premature contraction,non - sustained ventricular tachycardia and ventricular fibrillation of experiment group was statistically significantly lower than those of control group, respectively(P < 0. 05). There was interaction between time and method(P < 0. 05);main effects of time and method were significant(P < 0. 05);QT interval dispersion and QTcd of experiment group were statistically significantly lower than those of control group after 6 hours,24 hours,48 hours and 72 hours of PCI( P < 0. 05). Conclusion Alprostadil has certain cardioprotective effects in postoperative ASTEMI patients treated by emergency PCI,can effectively reduce the incidence of postoperative ventricular arrhythmias, its mechanism may relate to anti - inflammatory action, antioxygenation and microcirculation improving actions.
