摘要
目的:探讨结直肠癌的构成特点及变化趋势。方法:将2004年1月至2013年12月确诊并接受手术的结直肠癌患者划分为2004年-2008年和2009年-2013年两个阶段,回顾性分析其临床资料,包括年龄、性别、病理分型、病变部位及TNM/Dukes分期。结果:9 769例结直肠癌患者纳入研究,其中<40岁及≥60岁患者比例分别为5.1%、52.7%。前后5年相比,诊断时平均年龄从59岁延迟到61岁,<40岁的结直肠癌的构成比从6.3%降低到4.1%,≥60岁的构成比从49.6%上升到55.3%;直肠癌比例下降,左半结肠癌比例升高,且接受腹会阴联合直肠切除手术(Miles术)的直肠癌患者比例从28.0%降低到21.5%;TNMⅠ/Dukes A期患者所占的比例提高(12.1%vs 9.6%,P<0.001),而TNMⅣ/Dukes D期患者所占的比率有所下降(17.5%vs 15.7%,P=0.02)。结论:10年来,结直肠癌住院患者逐年增加,发病年龄趋于老年化,直肠癌患者比例呈下降趋势,早期诊治的结直肠癌的手术比例明显增加。
Objective： To explore the composition characteristics and variable tendency of colorectal cancer（ CRC）in Liaoning Caner Hospital. Methods： The clinical data of surgeric patients were retrospectively analyzed ,which diag-nosed CRC between January 2004 and December 2013 in our hospital,and were divided in to two groups ：2004 ？2008 and 2009 ？ 2013. Patient data was obtained,including age, gender, pathological classification, location of lesions, and TNM/Dukes stage. Results： A total of 9 769 CRC patients were included. The proportion of young（the age under 40 years old） and elderly（the age over多60 years old ） patient was 5. 1 % , 52. 7% respectively. The mean age of patient was delayed from 59 years old（2004 -2008） to 61 years old（2009 -2013） ,and the proportion of young patients de-creased from 6.3% to4.1%（P〈0. 001 ） ,but the elderly patients increased from 49. 6% to 55. 3% （ P 〈 0. 001 ）. The ratio of rectal cancer decreased slightly, and the left hemicolon cancer increased significantly. The percentageof rectal cancer received abdominoperineal resection（ Miles operation） decreased from 28. 0% to 21.5% （P 〈0. 001 ）. The proportion of TNM I /Dukes A stage in 2009 -2013（ 12.1% vs9.6%,P〈0. 001） increases, TNM IV/Dukes D stage was decreased slightly（ 17. 5% vs 15.7% = 0. 02） . Conclusion： Colorectal cancer inpatient increased gradu-ally in past decade,the age of onset has aging trend. The proportion of rectal cancer decreased,less patients received abdominoperineal resection （ Miles operation）. Early colorectal cancer surgery ratio increased in recent five years.
