锥形束CT结合冲击式气动手机在拔除上颌前部埋伏牙中的应用 被引量:7

摘要 目的探讨锥形束CT(CBCT)与冲击式气动手机相结合在拔除上颌前部埋伏牙中的优势。方法选择需要拔除上颌前部埋伏牙的患者30例,术前分别拍摄全景片与CBCT,术中采用冲击式气动手机微创拔除患牙,对全景片与CBCT两种定位方法进行比较,并记录手术时间,术中术后并发症发生率。结果与全景片比较,CBCT能准确定位上颌埋伏牙的位置方向,差异有统计学意义。微创拔牙手术时间为10-35 min,平均为16 min,较传统方法时间有所减少。患者手术中主观感觉较好,各种牙拔除术并发症发生率仅有6.7%。结论 CBCT可以更准确定位上颌前部埋伏牙。CBCT与冲击式气动手机相结合可以缩短手术时间并减少并发症的发生,值得临床进一步研究和推广。 Objective To explore the advantage of cone beam computed tomography( CBCT) combining impact air handpiece in exodontia of embedded supernumberary teeth located in maxillary anterior region and impacted canine. Methods Totally 30 cases were included. Excluding extraction contraindication,preoperative X-ray and CBCT examination were executed to find out the location of the impacted teeth. A minimally invasive operation approach was chosen to extract the embedded supernumberary teeth. Impact air handpiece was adopted during the operation. Intraoperative and postoperative feeling of the patients and the complications were recorded. Results There were statistical difference between CBCT and panoramic radiography examination( P〈0. 05). Minimally invasive extraction time lasted from 10 to 35 minutes,average 16 minutes,shorter than tranditional extraction. All patients maintained good feeling in the operation. None had serious reactions. Conclusions CBCT can accurately locate the impacted tooth in maxillary anterior region. CBCT combining with impact air handpiece provide a minimally invasive operation. The method achieved less uncomfortable feeling and reduced operation time,which is recommendable and worthy of further research.
