摘要
目的观察并总结系统性红斑狼疮并发急腹症的临床诊治方法。方法选择南乐县人民医院40例系统性红斑狼疮并发急腹症患者的临床资料,观察全部患者的临床症状表现,实验室检查、观察患者的活动性与急腹症的关系,给予对症治疗。结果 31例患者发生急腹症与红斑狼疮活动有关,9例与其他因素有关,两组患者红斑狼疮活动度平均分比较,具有明显差异（P〈0.05）。实验室检查发现,全部患者均补体C3降低;19例患者给予病理检查发现有增生、炎症改变。31例因红斑狼疮引发急腹症的患者经治疗1-12d后临床症状得到好转,9例因其他原因引发急腹症的患者中,1例因急性胰腺炎导致病情加重外,其他患者均获得好转。腹部CT检查阳性率为58.1%。结论系统性红斑狼疮合并急腹症患者应给予及时的CT及B超等检查,并给予红斑狼疮活动度检查,确认患者发生急腹症的原因后给予对症处理,以降低死亡率,改善预后。
Objective To observe and summarize the clinical diagnosis and treatment of systemic lupus erythematosus acute abdomen. Methods Totally Our hospital clinical data of 40 patients with systemic lupus erythematosus patients with acute abdomen, the clinical symptoms of all patients, laboratory tests, observe the relationship between patient mobility and abdomen, and given symptomatic treatment. Results Totally 31 cases of patients with acute abdomen and lupus - related activities, nine cases related to other factors, the two groups of patients with systemic activity average, compared with a significant difference （P 〈 0.05 ）. Laboratory tests found that all patients were reduced complement C3; 19 cases were treated with pathologic examination revealed hyperplasia, inflammatory changes. 31 cases caused due to lupus patients after treatment of acute abdomen after 1 - 12d clinical symptoms get better, because patients with other causes of acute abdomen caused nine cases, one patient with acute pancreatitis leading to exacerbations, other patients were better. Abdominal CT scan positive rate of 58.1%. Conclusion Systemic lupus erythematosus patients with acute abdomen should give timely CT and B ultrasound examination, and given the degree of disease activity in check, confirm the causes of acute abdomen of patients given symptomatic treatment, to reduce mortality and improve prognosis.
