摘要
目的探讨胱抑素C(Cys C)检测对急性肾损伤早期诊断的临床意义。方法选择急性肾损伤的患者90例为观察组,健康志愿者40例为对照组,比较两组血Cys C、肾小球滤过率(GFR)、血肌酐(SCr)、尿素氮(BUN)检测结果,分析Cys C与其他指标及患者病情程度间的相关性。结果观察组GFR显著低于对照组,Cys C、BUN、SCr均显著高于对照组(P<0.05)。随着急性肾损伤病情的加重,患者GFR呈降低趋势,而Cys C、BUN、SCr呈升高趋势(P<0.05)。观察组BUN、Scr水平与Cys C水平呈正相关性(r=0.767、r=0.721,P<0.05),GFR与Cys C水平呈负相关性(r=-0.732,P<0.05)。急性肾损伤后,Cys C平均升高时间为(1.1±0.4)d,而SCr为(3.2±1.1)d,两者有显著差异(P<0.05)。结论在急性肾损伤患者,Cys C与Scr、Bun及GFR有良好相关性,且Cys C升高时间早,可以用于急性肾损伤患者的早期诊断。
Objective To explore the clinical significance of cystatin C (CysC) detection in early diagnosis of acute renal injury. Methods 90 patients with acute renal injury were selected as an observation group and 40 healthy volunteers as a control group in order to compare the results of the blood CysC, GFR, SCr and BUN detection in the two groups to analyze the correlation between CysC and other indicators and severity of patient's condition. Reeules The GRF in the observation group was significantly lower than that in the control group, but the CysC, BUN and SCr were significantly higher than those in the control group (P 〈 0.05). With the aggravation of acute kidney injury, the GFR of patients showed a decreasing trend, but CysC, BUN and SCr showed an increasing trend (P 〈 0.05). The levels of BUN and Set were positively correlated to the CysC level in the observation group (r = 0.767, r = 0.721, P 〈 0.05), while the GFR level showed a negative correlation with and CysC level (r = -0.732, P 〈 0.05). After the development of acute kidney injury, the average rise time of CysC was (1.1±0.4) d and that of SCr(3.2±1.1) d; there was significant difference between the two indicators (P 〈 0.05). Conclusion For patients with acute kidney injury, CysC has good correlation with Scr, Bun and GFR and has short rise time, thus it is suitable for the early diagnosis of acute kidney injury.
