摘要
目的观察不同海拔地区健康人淋巴细胞亚群、血小板及凝血功能的改变。方法对西安(海拔400 m)、西宁(海拔2 300 m)以及兴海县(海拔4 000 m)健康体检人群进行静脉采血,测定血液中淋巴细胞亚群,血清免疫指标,血小板计数及凝血指标。结果西安组与西宁组及兴海组比较CD3^+、CD4^+、CD19^+淋巴细胞亚群均有统计学差异(P<0.05),且西宁组与兴海组比较也具有统计学差异(P<0.05);西安组、西宁组以及兴海组两两比较CD8^+淋巴细胞亚群均无统计学差异(P>0.05);兴海组与西安组及西宁组比较CD4^+/CD8^+有统计学差异(P<0.05),但西安组与西宁组比较CD4^+/CD8^+无统计学差异(P>0.05);西安组与西宁组及兴海组比较Ig G、Ig M及Ig A均有统计学差异(P<0.05),且西宁组与兴海组比较也具有统计学差异(P<0.05)。西安组与西宁组及兴海组比较CD62p、CD63细胞亚群及血小板计数均有统计学差异(P<0.05),且西宁组与兴海组比较也有统计学差异(P<0.05);西安组与西宁组及兴海组比较PT、APTT、FIB及TT均有统计学差异(P<0.05),且西宁组与兴海组比较也有统计学差异(P<0.05)。结论随着海拔的升高,氧浓度的下降,正常人群淋巴细胞亚群CD3^+、CD4^+及CD19^+百分比逐渐下降,CD4^+/CD8^+逐渐下降,血清免疫指标逐渐升高,血小板计数逐渐下降但血小板活化百分率逐渐升高,凝血指标PT、APTT、TT逐渐延长,FIB含量逐渐下降。高海拔生活的人群为适应低氧环境机体的部分指标发生了改变。
Objective To observe the changes in lymphocyte subgroup,platelet and coagulation function in healthy person at different altitudes. Methods The venous blood of healthy people from Xi’ an city( 400 m above sea level),Xining city( 2 300 m above sea level) and Xinghai county( 4 000 m above sea level) were collected. Then,the lymphocyte subsets,serum immunological indexes,platelet count and the coagulation index from samples collected at different altitudes were determined. Results Compared with Xining group and Xinghai group,the CD3^+,CD4^+,CD19^+lymphocyte subsets in Xi’an group were significantly different( P 〈 0. 05). Comparison between Xining group with Xinghai group also displayed significant difference( P 〈 0. 05). When comparing the CD8^+lymphocyte subsets in Xi’ an group,Xining group and Xinghai group,no significant difference was found( P 〉 0. 05). Compared with Xi’ an group and Xining group,the CD4^+/ CD8^+in Xinghai group was not significantly different( P 〈 0. 05),but no significant difference was found in Xi’an and Xining group( P 〉 0. 05). The Ig G,Ig M and Ig A in Xi’ an in all three groups showed significant differences( P 〈0. 05). Significant differences also were found when comparing Xining group with Xinghai group( P 〈 0. 05). The CD62 p,CD63 and platelet count in Xi’an group were significantly different from the Xinghai and Xining group( P 〈 0. 05),however significant difference could also be found when comparing Xining group with Xinghai group( P 〈 0. 05). The PT,APTT,FIB and TT in all three groups were significantly different( P 〈 0. 05). Lastly,significant differences were found in Xining group when compared with Xinghai group( P 〈 0. 05). Conclusion With the increase of altitude and decreased supply of oxygen,the lymphocyte subsets CD3^+,CD4^+and CD19^+drops gradually. In addition,the CD4^+/ CD8^+drops gradually.The serum immunological indexes rises gradually. The platelet count drops gradually but the percentage of activated platelets rises gradually as well. The PT,APTT and TT are prolonged gradually and the FIB drops gradually. The people living at high altitude change their physiological indices in order to adapt to hypoxic environment.
