摘要
目的探讨北京市通州社区T2DM患者的血糖控制与服药依从性和自我管理现状,以及相关性。方法对纳入研究的318例T2DM患者进行问卷调查和抽血体检。以HbA_1c作为血糖控制指标,8条目Morisky服药依从性问卷(MMAS-8)和糖尿病自我管理行为量表(SDSCA)得分作为服药依从性和自我管理指标。结果 HbA_1c平均水平为(7.80±1.53)%,控制率(HbA_1c<7%)为34.9%。MMAS-8为(5.99±1.88)分,50.9%的T2DM患者有较好服药依从性;SDSCA为(34.84±13.86)分。19.8%的T2DM患者自我管理总体情况良好。服药依从性越好,自我管理情况越好,病程<5年者,HbA_1c水平就越低。结论北京市通州社区T2DM患者HbA_1c控制水平不佳,服药依从性不高、自我管理总体情况较差。提高患者服药依从性和自我管理水平,特别是运动方面的自我管理,是提高HbA_1c控制水平的有效方法。
Objective To survey the status quo of glycemic control,medication adherence and self- management among patients with diabetes in community and to investigate the association between medication adherence combined with self-management behaviors and glycemic control. Methods Questionnaire, physical examination and glycated hemoglobin test were conducted in diabetic patients from Tongzhou community in Beijing. HbA1 c〈7% was used as indicator for good glycemic control. Medication adherence was evaluated by 8-Item Morisky Medication Adherence Scale (MMAS-8) and self-management behavior was evaluated by The Summary of Diabetes Self-Care Activities Measure (SDSCA). Results Total 318 patients were included in the analysis. The mean HbA1 c level was (7.80± 1.53) %,with 34. 9% reached the target of HbA1 c%7%. The average score of MMAS-8 was (5.99 ± 1.88), with 50. 9% showed optimal adherence. The average total score of SDSCA was (34. 84± 13.86), with 19.8% presented good self-management behavior. Higher MMAS-8 score (indicating better medication adherence), higher score in exercise dimension in SDSCA (indicating better self-management) and diabetes duration %5 year were associated with lower HbA1 c level. Conclusion The study reveals low HbA1 c control level, poor medication adherence and self-management among patients with diabetes in community. Improvement of medication adherence and self-management (exercise in particular) may be an effective approach to improve the glycemic control.
