Study on antimicrobial resistance from the People's Hospital of Tibet Autono-mous Region in 2015

摘要 目的分析西藏自治区人民医院2015年度细菌分布及耐药情况。方法采用纸片扩散法(Kirby-Bauer法)或ATB检测法,测定监测细菌对抗菌药物敏感性,用WHONET 5.6软件进行数据分析。结果共有2096株非重复细菌进行药敏试验,包括革兰阴性菌1628株(77.7%)和革兰阳性菌468株(22.3%)。耐甲氧西林金黄色葡萄球菌(MRSA)和耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)检出率分别为33.9%和62.7%。未检出万古霉素耐药粪肠球菌和屎肠球菌。革兰阴性菌中分离率前三位分别为大肠埃希菌(31.2%)、克雷伯菌属(25.9%)和鲍曼不动杆菌(15.5%)。大肠埃希菌对头孢噻肟和环丙沙星的耐药率分别为42.6%和56.7%。肺炎克雷伯菌对亚胺培南耐药率为1.6%。铜绿假单胞菌对碳青酶烯类、哌拉西林/三唑巴坦、头孢吡肟、头孢他啶、氨基糖苷类(阿米卡星和庆大霉素)和喹诺酮类(环丙沙星和左氧氟沙星)的耐药率均低于30%。鲍曼不动杆菌对碳青霉烯类药物耐药超过65%。结论西藏代表性医院的革兰阴性菌耐药状况较严重,鲍曼不动杆菌的耐药状况尤为突出。 Objective To explore the antimicrobial resistance of common clinical strains in the People's Hospital of Tibet Autono-mous Region in 2015. Methods All the objective bacterial isolates were collected and identified by unified protocol, then the susceptibility date were analyzed by software WHONET 5.6 which was detected with the method of disc-diffusion or ATB systems. Results 2,096 isolates and their antibacterial susceptibility data were acquired, which included 1628 (77.7%) Gram-negative bacillus and 468 (22.3%) Gram- positive coccus. The isolating rates of methicillin-resistant Staphylococcus aureus (MRSA) and methicillin-resistant coagulase negative Staphylococcus (MRCNS) were 33.9% and 62.7%, respectively. No vancomycin and linezolid non-sensitive Staphylococcus isolates were found, and no isolates of E. faecal& and E. faecium was resistant to vancomycin, teicoplanin and linezolid. The top three populations of Gram-negative baeterial isolates were E. coli (31.2%), Klebsiella (25.9%), A. baumannii (15.5%), respectively. 42.6% and 56.7% E. coli were resistant to cefotaxime and ciprofloxacin, respectively. 1.6% Klebsiella pneumoniae were resistant to imipenem. The resistant rate of P aeruginosa to carbapenems, aminoglycosides, piperacillin/tazobactam, cefepime, cefoperazone/sulbactam, ceftazidime were less than 30%. The resistance rates of A. baumannii against carbapenem were above 65.0%. Conclusion The drug resistance situation of Gram-negative bacillus are severe in Tibet, particular in A. baumannii.
