摘要
目的探究分析16层螺旋cT机行颅脑非螺旋扫描成人颅脑的条件选择。方法随机选取2013年2月至2015年2月于本院体检中心体检的500名健康成人作为研究对象,将其随机均分为5组,对其用16层螺旋CT机行颅脑非螺旋扫描,对第2~5组采用低剂量(200、160、100、80mAs,120kW)扫描,对第1组采用常规剂量(颅顶部200mAs、颅底部240mAs,120kW)扫描,结合对5分制的运用,安排2名放射科医师独立对各组图像主观评分,在评价结果一致性的评定方面运用Kappa一致性检验,记录平均剂量长度乘积(DLP)值和各单次加权CT剂量指数(CTDIv01),计算相对第1组,第2~5组的CTDIvol及DLP值下降的比例。结果经本院后续统计分析得出,2名医师评分的一致性好(P=0.017,K=0.860);图像质量主观评分与扫描剂量呈正相关(P〈0.05,r=0.512),在第2~4组中,图像质量优良者(评分〉3分)所占比例达95%~98%,相比于第1组,二者对比差异无统计学意义(P〉0.05);相比于第1组,第2~5组的DLP和CTDIv01分别降低20.68%、36.50%、60.40%、68.23%和15.79%、32.59%、57.89%、66.40%,第1组与各组的DLP值对比,差异具有统计学意义(P〈0.05)。相比于第1组的图像质量,第5组图像质量下降明显,评分〉3分的图像质量占68.23%,2组对比,差异具有统计学意义(P〈0.05)。结论经本院研究得出,对成人颅脑行颅脑非螺旋扫描时,管电流采用100mAs,可显著降低低剂量CT扫描的辐射剂量,不会明显影响到对成人颅脑解剖结构的观察和诊断。
Objective To explore and analyze the conditions for non-helically scanning adult brain by 16 slice CT. Methods 500 healthy adults taking physical examination at the physical examination center of our hospital fi'om February, 2013 to February, 2015 were randomly selected as study objects and randomly divided into 5 groups. All the objects were non-helically scanned the brains by 16 slice CT with low doses (200, 160, 100, and 80 mAs, 120kW) for group [I , group HI , group 1V, and group V and normal dose (200 mAs for cranium and 240 mAs for skull base, 120 kW) for group Ⅰ . The images of all the groups were subjectively scored by 2 radiologists with 5-point system independently. The consistency of the results was tested by Kappa consistency. The average dose length product (DLP) and the single value weighted CT dose index (CTDIvol) were recorded. The decrease proportions of CTDIvol and DLP in group Ⅱ, group Ⅲ, group Ⅳ, and group V compared with those in group Ⅰ were calculated. Results The consistency in the scores between these two radiologists was good (P-=-0.017, K=0.860 ). The image quality score positively correlated with scanning dose (P〈0.05, r=0.512). 95%-98% images had excellent quality (score 〉 3 points) in group Ⅱ , group Ⅲ, group Ⅳ, and group V, with no statistical differences as compared with those in group 1 (P 〉 0.05). Compared with group I , the DLP and CTDIvol decreased 20.68% and 15.79%, 36.50% and 32.59%, 60.40% and 57.89%, and 68.23% and 66.40% in group Ⅱ, group Ⅲ , group IV, and group V , respectively, with statistical differences (P〈0.05). The image quality was poorer in group V (only 68.23% images' scores were over 3) than in group I , with a statistical difference (P 〈 0.05). Conclusions Non-helically scanning adult brain by 16 slice CT with a tube current of 100 mAs can significantly reduce the radiation dose of low dose CT scanning and does not significantly affect the anatomical structure observation and the diagnosis of adult brain.
