摘要
目的了解医院感染中广泛耐药鲍氏不动杆菌的同源性及耐消毒剂基因qacEΔ1的携带情况,从而针对性地制定预防控制措施,有效预防医院感染。方法收集2014年住院患者中分离的15株泛耐药鲍氏不动杆菌,采用肠杆菌科基因间重复序列-聚合酶链反应(ERIC-PCR)进行同源性分析,采用聚合酶链反应(PCR)检测其耐消毒剂基因qacEΔ1携带情况。结果 15株医院感染广泛耐药鲍氏不动杆菌经ERIC-PCR分型分A、B两种基因型,A型10株,B型5株;ICU分别分离出A型6株、B型3株;A型其余4株菌株分别分布于PICU(儿科ICU)、整形外科病房、心血管外科病房及神经外科病房;B型其余2株菌株分别分布于胃肠外科病房及骨外科病房;15株医院感染广泛耐药鲍氏不动杆菌均携带耐消毒剂基因qacEΔ1。结论医院感染广泛耐药鲍氏不动杆菌存在菌株的A/B型的克隆流行,其主要分布在基础病情严重、侵入性操作频繁的ICU;医院感染广泛耐药鲍氏不动杆菌耐消毒剂基因qacEΔ1携带率极高。
OBJECTIVE To understand the homology of pandrug-resistant Acinetobacter baumannii causing nosocomial infection and the disinfectant resistance gene qacEΔ1so as to put forward targeted prevention measures and effectively prevent the nosocomial infection.METHODS Totally 15 strains of pandrug-resistant A.baumannii were isolated from the hospitalized patients in 2014,then the homology was analyzed by using Enterobacteriaceaerepetitive intergenic sequences-polymerase chain reaction（ERIC-PCR）,and the disinfectant resistance gene qacEΔ1was detected with the use of polymerase-chain-reaction（PCR）.RESULTS The 15 strains of pandrug-resistant A.baumannii were divided into the genotype A（10strains）and the genotype B（5strains）through ERIC-PCR.Totally6 strains of genotype A and 3strains of genotype B were isolated from ICUs,the rest of 4strains of genotype A respectively distributed in the PICU（pediatric ICU）,plastic surgery wards,cardiovascular surgery wards,and neurosurgery wards.The rest of 2strains of genotype B distributed in the gastrointestinal surgery wards and bone surgery wards.All of the 15 strains of pandrug-resistant A.baumannii causing nosocomial infection carried with the disinfectant resistance gene qacEΔ1.CONCLUSIONThe genotype A/B pandrug-resistant A.baumannii is prevalent in the hospital and mainly distributes in the ICUs where the underlying diseases are severe and the invasive operations are frequent.The carrying rate of the disinfectant resistance gene qacEΔ1is extremely high in the pandrug-resistant A.baumannii causing nosocomial infection.
