摘要
目的探讨MRI动态对比增强联合扩散加权成像(DWI)对孤立性肺结节诊断价值。方法选取2013年6月—2015年6月期间以孤立性肺结节为首诊表现入院的患者56例,进行MRI动态对比增强联合DWI诊断,并参考病理诊断结果,分析该诊断方式的敏感性、特异度、准确性、阳性预测值及阴性预测值。结果在b=600 s/mm2时,符合富血供结节患者的良、恶性结节之间ADC值差异比较有显著差异(P<0.05);MRI动态对比增强联合DWI诊断孤立性肺结节灵敏度为86.96%(20/23)、特异度为81.81%(9/11)、准确性为85.29%(29/34)、阳性预测值为90.91%(20/22)及阴性预测值为75%(9/12)。结论 MRI动态对比增强联合DWI诊断孤立性肺结节灵敏度、特异度、准确性、阳性预测值及阴性预测值均较高,有利于对良、恶性孤立性肺结节准确诊断,对孤立性肺结节诊断有重大价值。
Objective To investigate the value of dynamic contrast enhanced MRI combined with diffusion weighted imaging(DWI) in diagnosis of solitary pulmonary nodules. Methods Fifty-six patients with initial diagnosis of solitary pulmonary nodules hospitalized between June 2013 and June 2015 were selected. The diagnosis was made by dynamic contrast enhanced MRI combined with diffusion weighted imaging, and its sensitivity, specificity, accuracy, positive predictive value and negative predictive value in diagnosis were determined with reference to pathological results. Results While b=600 s/mm^2, there were significant differences in ADC values between benign and malignant nodules with rich blood supply (P〈0.05).The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of dynamic contrast enhanced MRI combined with DWI in diagnosis of solitary pulmonary nodules were 86.96% (20/23) , 81.81% (9/11) , 85.29% (29/34) , 90.91% (20/22) and 75% (9/12) , respectively. Conclusion Dynamic contrast enhanced MRI combined with DWI is favorable for the diagnosis of benign and malignant solitary pulmonary nodules with higher sensitivity, specificity, accuracy, positive predictive value and negative predictive value.
