摘要
目的：观察强心宁煎剂联合曲美他嗪对慢性心衰左心室功能及血浆心钠素、内皮素水平的影响。方法：选取2013年2月~2015年1月我院心内科收治的62例慢性心衰患者为研究对象,随机数字表法分为研究组和对照组各31例,对照组给予强心宁煎剂治疗,研究组在对照组基础上另联合曲美他嗪治疗,对两组治疗后临床疗效、左心室功能[左心室射血分数（LVEF）、左室舒张末期内径（LVDd）、左室收缩末期容量（LVESV）]、血浆心钠素（ANP）、血浆内皮素（ET）及药物不良反应进行观察比较。结果：两组治疗后研究组临床总有效率87.10%较对照组62.07%显著高（P〈0.05）;两组治疗后LVEF较治疗前显著升高,LVDd、LVESV显著下降,且治疗后研究组LVEF较对照组升高显著,LVDd、LVESV降低显著,均具有统计学意义（P〈0.05）;两组治疗后ANP、ET分别均较治疗前降低,仅ANP治疗前后有显著差异（P〈0.05）,且治疗后研究组ANP、ET分别均较对照组降低显著,均具有统计学意义（P〈0.05）;两组药物不良反应比较无显著差异（P〉0.05）。结论：强心宁煎剂联合曲美他嗪对慢性心衰患者具有较好的临床疗效,且对左心室功能、血浆心钠素、内皮素水平均具有显著改善作用。
Objective： To observe the effects of Qiangxinning Decoction combined with trimetazidine on left ventricular function,levels of plasma atrial natriuretic peptide（ ANP） and endothelin（ ET） in patients with chronic heart failure. Methods： Sixty-two patients with chronic heart failure who were admitted to the department of cardiology in our hospital between February2013 and January 2015 were randomly divided into study group and controlled group by the random number table method,with31 cases in each group. The controlled group was treated with Qiangxinning Decoction while the study group,based on the controlled group was treated with trimetazidine. After treatment,the clinical curative effect,left ventricular function [left ventricular ejection fraction（ LVEF）,left ventricular end diastolic diameter（ LVEDD）,left ventricular end systolic volume（ LVESV） ],plasma ANP,plasma ET and adverse drug reactions were observed and compared between the two groups. Results： After treatment,the total response rate in the study group（ 87. 10%） was higher than that in the controlled group（ 62. 07%）（ P〈0. 05）;after treatment,compared with those before treatment,LVEF was significantly higher while LVEDd and LVESV were significantly lower. After treatment,LVEF of the study group was significantly higher than the controlled group while LVEDd and LVESV were significantly lower than the controlled group（ P〈0. 05）; after treatment,ANP and ET in the two groups were lower than those before treatment but there only was significant difference in ANP before and after treatment（ P〈0. 05）. After treatment,ANP and ET were significantly lower than the controlled group（ P〈0. 05）; there was no significant difference in adverse drug reactions between the two groups（ P〉0. 05）. Conclusion： The effect of Qiangxinning Decoction combined with trimetazidine in the treatment of patients with chronic heart failure is good. It also can effectively improve left ventricular function and levels of plasma ANP and ET.
