摘要
目的分析乌鲁木齐市社区全科医师签约制服务发展现状,探讨乌鲁木齐市全科医师签约制服务发展的相关对策。方法通过整群抽样方法从乌鲁木齐市7个区59个社区卫生服务中心中随机抽取5个符合标准的社区卫生服务中心,分别对303名社区医务人员和3 762名社区居民展开问卷调查,了解乌鲁木齐社区全科医师签约制服务发展现状。结果医务人员对签约制服务的知晓率为88.0%,64.4%的医务人员愿意开展签约制服务。46.0%的社区所辖居民知晓全科医师签约制服务,80.6%的社区居民愿意开展此项服务。全科医师签约制服务的相关因素按重要程度进行排名,最重要的是获得政策支持(44.2%)。33.7%的医务人员参加过健康管理师职业技能培训,仅28.7%的医务人员有过带教经历,29.1%的社区居民把全科医师服务水平作为签约制服务的主要因素。结论社区医务人员及所辖居民对签约制服务意愿尚可,政策支持、技能培训及资源配置是影响医务人员的主要因素,全科医师服务水平是影响社区居民签约制服务的首要因素,应切实优化资源配置、配套激励机制;加强宣传教育,建立医院-社区联动服务。
Objective To analyze the development present situation of community general practitioner contract system service in Urumqi,and to discuss the countermeasure research of the development of the general practitioner contract system in Urumqi. Methods 5 communities were selected randomly by cluster sampling method from the59 health service centers in Urumqi,and than expanded a questionnaire to community medical staff and community residents,to further understand the development present situation of community general practitioner contract system service in Urumqi. Results 303 community medical staff and 3 762 residents who under the jurisdiction of the community,the awareness rate of contract service of medical staff was 88 %; 64. 4 % medical staff wanted to carry out contract system services. 46 % community residents are awared of the general practitioner contract system services,80. 6 % wanted to carry out this service. According to the importance to rank the related factors of the general practitioner contract system,the most important was to get policy support 44. 2 %. In the knowledge and skills training for community physicians,this study showed that 33. 7 % of the medical staff participated in health management skills training,and only 28. 7 % of the medical staff had a teaching experience,29. 1 % community residents took the general practitioner service level as the main factors of contract system. Conclusion Community medical staff and residents under the jurisdiction satisfied with contract system,and policy support,skill training and resource allocation are the main factor that affects the medical staff. general practitioner service level is the primary factor on the impact of community residents contract system services,we should optimize the allocation of resources,support incentive mechanism,strengthen publicity and education and establish hospital- community service.
