摘要
目的研究老年单纯收缩期高血压患者24 h平均脉压与肾脏功能之间的相关性,为临床提供依据。方法选老年单纯收缩期高血压患者158例,应用动态血压监测技术测定24 h平均脉压,并同时检测血肌酐、肾小球滤过率、24小时尿微白蛋白。结果 24 h平均脉压越大,肾脏功能越差(P<0.05或P<0.01),平均脉压与左肾小球滤过率(r=-0.30,P<0.05)负相关,与24小时尿微白蛋白及肌酐水平(r=-0.32,P<0.05)呈正相关。结论老年单纯收缩期高血压患者肾脏功能与24 h平均脉压呈负相关,后者可作为肾脏功能受损的预测因子之一。
Objective To investigate whether there is correlation between 24-hour ambulatory average pulse pressure and renal function in elderly patients with isolated systolic hypertension. Methods 158 patients with elderly isolated systolic hypertension were enrolled. Mean pulse pressure was determined by 24 hours ambulatory blood pressure. Renal function was evaluated by measuring serum creatinine, Glomerular ifltration rate(GFR) and 24-hour microalbuminuria. Results The greater the mean pulse pressure was, the worse the renal function was (P〈0.05 or P〈0.01). Renal function represented by GFR was negatively correlated with mean pulse pressure of 24 hours ambulatory blood pressure (r=-0.30, P〈0.05). 24-hour microalbuminuria (r=-0.32, P〈0.05) was positively correlated with 24-hour ambulatory mean pulse pressure. Conclusion The 24 hours ambulatory mean pulse pressure was an important predicting risk factor for kidney dysfunction in elderly isolated systolic hypertension.
