II型糖尿病尿蛋白含量与黄斑区视网膜厚度的相关性研究 被引量:5

摘要 目的探讨在II型糖尿病患者中不同程度蛋白尿与黄斑区视网膜厚度之间的相关性。方法回顾性系列病例研究。选择2014年9月至2015年9月西安交通大学医学院第二附属医院眼科门诊和内分泌科会诊诊断为糖尿病性视网膜病变(DR)III、IV期的52例II型糖尿病患者的临床资料,记录患者的全身检查情况和疾病史,检查所有患者的彩色眼底照相、相干光断层扫描(OCT),荧光素眼底血管造影(部分患者)检查。根据糖尿病患者的尿蛋白含量将其分为微量蛋白尿组和大量蛋白尿组,两组中与糖尿病及肾功能有关的因素采用两独立样本的t检验和卡方检验;组间有差异的因素与黄斑区视网膜厚度的关系采用Spearman相关;与黄斑视网膜厚度有关的因素采用线性回归的方法,求出其与黄斑区视网膜厚度的关系,并做出散点图及线性回归模型。结果II型糖尿病患者的尿蛋白含量与黄斑区视网膜厚度呈线性正相关(r 〉0);这些患者黄斑的水肿从形态上可分为视网膜海绵样肿胀、黄斑囊样水肿、神经上皮浆液性脱离以及三种形态的混合。结论蛋白尿是糖尿病性黄斑水肿的独立危险因素。 Objective To investigate the association between the urinary protein content and the macular retinal thickness in cases with type II diabetes. Methods Retrospective case series study. 52 cases with type II diabetes from September 2014 to September 2015 were selected and their clinical information was collected. These cases were diagnosed with diabetic retinopathy (III or IV phase) in the Department of Ophthalmology and Endocrinology. The result of body examination was recorded and all the patients were examined documenting spectral domain optical coherence tomography (SD-OCT) and color fundus photography. Some of them were also examined by fluorescence fundus angiography. According to 24 hour urinary protein content, samples were divided into microalbuminuria group (≤300mg/d) and macroalbuminuria group (〉300mg/d). Independent-Samples T test and Chi-square test were used to make comparisons between pairs of groups. Correlations between variables were investigated by Spearman rho correlation test. Significant variables from Spearman's correlation analyses were ftLrther analyzed by linear regression. When working out the correlation between the urinary protein content and the macular retinal thickness, the further research was done by scatter diagram and regression model. Results The relationship was linear positive correlation between urinary protein content and the macular retinal thickness in cases with type II diabetes. The images of OCT in macular edema include sponge-like retinal swelling, macular cystoid edema, serous retinal detachment and mixture of the above forms. Conclusions Urinary protein content is an independent risk factor of diabetic macular edema.
