“跳跃性”腰椎间盘退变 被引量:2

摘要 腰背痛是困扰全球的健康问题,使各国人民蒙受巨额的经济损失,随着人口的老年化和时间的推移,在未来的几十年,其患病率将会大幅上升。腰背痛的发病原因复杂多样,其中腰椎间盘退变扮演着一个重要的角色。目前有关腰椎间盘退变机制主要集中在椎间盘内营养物质合成和代谢途径受损、基质降解酶合成和代谢失衡、生物力学的改变、细胞因子等方面。近年来,有研究提出一种新的腰椎间盘退变模式——"跳跃性"腰椎间盘退变,其主要是在多个退变的椎间盘中间存在正常健康的椎间盘。它的发现为椎间盘退变机制提供了新的思路,同时也让临床医师思考如何合理选择手术方案,从而更好的指导临床。 Low back pain (LBP) is a global health problem which causes great economic burdens, and the prevalence rate will definitely rise sharply quickly with aging tendency of pupnlation. It has been demonstrated that disc degeneration plays a sig- nificant role in LBP, but the mechanism is still not well understood. It is found that nutrition, abnormal matrix degradation, bi- omechanics, cytokines expression are influencing factors to induce disc degeneration. So, understanding the mechanism of disc degeneration will provide further options for treating LBP. Recently, several studies have determined that the discs were able to degenerate at "skipped" levels in the lumbar spine, in this pattern, one or more non -degenerative discs exist among the de- generated discs. This review hence summarizes the research findings regarding disc degeneration at "skipped" levels. The results may help to explore the knowledge of disc degeneration, and to find out much better effective surgical treatments.
