摘要
目的分析神经眼科专科病房患者病因及病变部位分布情况。方法对2010年1月-2014年12月解放军总医院神经眼科住院患者的临床资料进行回顾性分析。结果共2 673例纳入研究,其中男性1 365例,女性1 308例,男女比例1.04：1。平均年龄（37.91±18.12）岁,其中20～60岁1 881例,占70.37%;病变部位分布：视觉传入神经系统2 376例（88.88%）,运动传出神经系统190例（7.11%）,眶周及颅内血管及软组织疾病107例（4.00%）。病因分布：视神经炎性脱髓鞘疾病845例（31.60%）,外伤性视神经病变282例（10.55%）,缺血性视神经病变236例（8.23%）,颅内肿瘤继发视神经萎缩207例（7.74%）,病因未明219例（8.19%）。结论本院神经眼科住院疾病群体以60岁以下中青年人群为主;病变性质以视觉传入神经系统障碍为主,视神经炎性脱髓鞘疾病、外伤性视神经病变、缺血性视神经病变及遗传性视神经病变是主要病因。
Objective To investigate the distribution of etiologic factors and the location of neuro-ophthalmic disorders of patients in Chinese PLA General Hospital. Methods Clinical data about hospitalized patients with neuro-ophthalmic diseases in department of neuro-ophthalmology of Chinese PLA General Hosptial from January 2010 to December 2014 were collected and retrospectively analyzed. Results A total of 2 673 patients with average age of（37.91±18.12） years（1 365 males and 1 308 females with the ratio of 1.04：1） were enrolled in this study, of which, 1 881（70.37%） patients aged over 20 years and below 60 years（including 60）. Distribution of lesion location： afferent visual system in 2 376（88.88%） patients, efferent visual system in 190（7.11%） patients, and orbital and intracranial vascular and connective tissue diseases in 107（4.00%） patients. Distribution of etiology： the inflammatory demyelinate optic neuritis was found in 845（31.6%） cases, traumatic neuropathy in 282 cases（10.55%）, ischemic optic neuropathy in 236 cases（8.23%）, optic atrophy secondary to intracranial tumor in 207 cases（7.74%）, and 219（8.19%） cases remained uncertain. Conclusion Majority of patients with optic neuropathy are young and middle-aged adults with age less than 60-yearold. The main pathogenesis is dysfuction in afferent visual system, and optic nerve is the most frequent lesion location. Inflammatory demyelinate optic neuritis, ischemic optic neuropathy, traumatic neuropathy and hereditary optic neuropathy are the main causes.
