摘要
目的:探讨多层螺旋CT(multislice CT,MSCT)三维重建在食管异物中的临床应用价值。方法:对21例食管异物患者,采用MSCT 5 mm扫描,0.625 mm重建。将扫描所得原始数据通过网络传送至后处理工作站,采用aw4.3平台上3D后处理软件进行重建,主要重建方法包括多平面重建(multiplan reformation,MPR)、最大密度投影(maximum intensity projection,MIP)及容积再现(volume rendering,VR)。结果:食管异物位于颈部的16例,位于胸部的5例。异物为条状、弧状、小片状或不规则状;异物在食道内走向多为斜向,其次为纵向或横向;5例穿透食管壁。CT能明确显示异物与食管周围组织的关系。结论:螺旋CT三维重建能明确显示食管异物大小、形态、位置及有无穿孔等并发症,MPR、MIP、VR 3种重建模式的相互结合对诊断食管异物及指导临床及时准确地制订治疗方案有明显优势。
Objective To explore the clinical application value of multislice CT 3D reconstruction in diagnosing esophageal foreign bodies.Methods Totally 21 cases with clinically confirmed esophageal foreign bodies were processed with multiplan reformation(MPR),maximum intensity projection(MIP),and volume rendering(VR),with collimation being 5 mm and reconstruction interval being 0.625 mm.CT data were transferred to a computer workstation through the network,and reconstructed using the software on windows aw4.3.Results There were 16 cases had the esophageal foreign bodies located at cervical esophagus and 5 ones had the bodies at thoracic esophagus.The bodies were often strip shaped,arc shaped,flakey or abnormally shaped,which were oblique,longitudinal or lateral in the esophagus.There were 5 patients with the bodies penetrating the esophagus wall.CT could display the relationship between the foreign body and the surrounding tissues of the esophagus definitely.Conclusion MSCT can display clearly the size,shape,location and complications such as perforation of the foreign bodies,and combined reconstruction modes of MPR,MIP and VR facilitate the diagnosis of esophageal foreign bodies and the direction to clinical treatment.
