摘要
目的比较采用小切口髌骨内侧旁(MMP)入路和小切口股内侧肌(MMV)入路行全膝关节置换术的效果。方法计算机检索Pub Med、万方、知网、维普等数据库,纳入对比经MMV入路和MMP入路行全膝关节置换术效果的文献,对纳入献进行质量评价,利用Rev Man软件5.3进行Meta分析。结果共纳入8篇相关文献,其中MMP组274例,MMV组272例。Meta分析结果显示,两组术后引流量、术后下肢力线情况比较,差异无统计学意义(P>0.05);MMP组手术时间、皮肤切口长度均短于MMV组(P<0.05);MMV组术后视觉模拟疼痛评分、外侧支持带松解率均优于MMP组(P<0.05),术后开始下地行走时间、开始直腿抬高时间早于MMP组(P<0.05),术后6周膝关节活动度大于MMP组(P<0.05)。结论采用MMP入路行全膝关节置换术手术时间及皮肤切口长度较MMV入路短,但采用MMV入路时患者膝关节功能恢复快、术后疼痛轻、术后并发症少。
Objective To compare the efficacy between mini-medial parapatellar( MMP) approach and mini-midvastus( MMV)approach for total knee arthroplasty( TKA). Methods The databases of computer retrieve included Pub Meb,Wanfang Database,CNKI,VIP. And the literatures about comparison of efficacy between MMP approach and MMV approach for TKA were collected. The qualities of included literatures were assessed. Then a Meta-analysis was conducted using Rev Man 5. 3 software. Results Eight relative literatures were involved,which consisted of 274 cases in the MMP group and 272 cases in the MMV group. The result of Meta-analysis showed that there were no significant differences in postoperative drainage volume or postoperative limb alignment between the two groups( P〉0. 05).The operative time and length of skin incision in the MMP group were shorter than those in the MMV group( P〈0. 05). Compared to the MMP group,the Visual Analog Scale score and the rate of lateral retinacular release after operation were superior,the time for walking down or straight leg raise after operation was earlier,and the range of knee motion after 6 weeks of operation was larger in the MMV group( P〈0. 05). Conclusion For TKA,MMP approach can obtain shorter operative time and length of skin incision. However,the patients undergoing TKA can acquire better recovery of knee function,less postoperative pain and complications when MMV approach is applied.
