摘要
【目的】分析遗传性非息肉病性大肠癌(hereditary non-polyposis colorectal cancer,HNPCC)的病理特点,为临床诊治提供可靠的参考依据。【方法】于2011年1月至2015年12月,选取我院收治的500例大肠癌患者作为此次研究的对象,对这500例患者的临床资料进行回顾性分析,对其病理特点进行总结分析。根据Amsterdam诊断标准、日本标准,对其中HNPCC患者、具有家族大肠癌病史的患者进行统计,并对HNPCC患者的病理特点进行总结分析。【结果】大肠癌的肿瘤位置分布于直肠、远端结肠、近端结肠,病理组织学分型为腺癌、粘液癌、未分化癌,伴有息肉的患者所占比例为17.4%。符合Amsterdam标准的HNPCC所占比例为1.8%,符合日本标准的HNPCC所占比例为3.4%,差异无统计学意义(P>0.05)。HNPCC主要集中于中年人群,肿瘤分布于近端结肠的所占比例最高,多为腺癌,伴有息肉的概率较小。HNPCC与普通型大肠癌的MLH1和MSH2蛋白阳性表达率比较,差异均具有统计学意义(P<0.05)。【结论】HNPCC的病理特点较为特殊,可根据其病理特点进行鉴别诊断。
【Objective】To study the proportion of hereditary non-polyposis colorectal cancer in patients with colorectal cancer and its pathological characteristics.【Methods】A total of 630 cases with colorectal cancer were selected in our hospital from January 2011 to December 2015. Their clinical data were retrospectively analyzed to find the cases in accord with the diagnostic criteria of HNPCC. The pathological characteristics of these patients were studied.【Results】There were 12 patients in accord with Amsterdam criteria, and the proportion was 1.90%. The pathological characteristics of HNPCC were as follows： the colorectal tumor occurred mostly in the right colon; the cancer type was primary colorectal cancer; all belonged to family colon cancer. In the 20 HNPCC cases according to the Japanese standard, the minimum age was 42 years old, the maximum age was 56 years old, and the average age was（46.3＋2.5） years.There was 1 case with asynchronous multiple primary carcinoma and 1 patient with synchronous multiple primary cancer. As for the tumor location, there were 9 cases in distal colon, 3 cases in rectum and 8 cases in proximal colon. There were 2 patients accompanied with tumor polyps.【Conclusion】The proportion of HNPCC is low in colorectal cancer. Compared with common colorectal cancer, the pathological features of HNPCC are more special.
