摘要
目的探讨血清叶酸和铁蛋白作为多发性骨髓瘤(MM)患者化疗前后病情检测指标的参考价值。方法回顾性选择该院MM患者109例为实验组,选择该院健康体检者101例为对照组。实验组检测其化疗前后、复发时的血清叶酸和铁蛋白,并进行化疗前后两组间比较分析。结果初治MM患者叶酸水平下降,铁蛋白水平升高,与对照组比较差异有统计学意义(t=6.12,P<0.05;t=59.08,P<0.05);完全缓解后叶酸及铁蛋白水平接近正常(P>0.05);随着病程的逐步缓解,叶酸水平接近正常范围(F=7.18,P<0.05);复发时叶酸水平下降,铁蛋白水平再次升高,与完全缓解时比较,差异有统计学意义(t=6.36,P<0.05;t=59.88,P<0.05)。结论化疗前后血清叶酸、铁蛋白水平的变化有助于对病情及预后做出判断;对MM复发具有早期预测价值。
Objective To explore the reference value of serum folic acid and serum ferritin in patients with multiple myeloma（MM） before and after chemotherapy. Methods Totally 109 patients with MM were chosen as the study group; 101 cases having physical examination in our hospital were chosen as the control group. The serum folate and ferritin before and after chemotherapy and at recurrence were tested and compared with the values in the control group. Results Folate level significantly decreased（t = 6.12, P〈0.05）while ferritin level significantly increased（t = 59.08, P〈0.05） in the patients with newly-diagnosed MM compared with the control group. After complete remission, the folate and ferritin levels of the patients returned to nearly normal levels（P〈0.05）. With the gradual remission of MM, folic acid got close to the normal range（F = 7.18, P〈0.05）; at relapse folate level significantly decreased（t = 6.36, P〈0.05） and ferritin significantly increased again（t = 59.88, P〈0.05） when compared to those at complete remission, indicating folate and ferritin level changes before and after chemotherapy have predictive value for short-term effect. Conclusions Changes in serum folate and ferritin levels before and after chemotherapy are helpful in estimation of patients＇ condition and prognosis and have predictive value for early relapse of MM.
