摘要
目的探讨解冻周期胚胎移植位置对妊娠结局的影响。方法回顾性分析2014年9月至2015年7月在山西医科大学第一医院生殖科行冻融胚胎移植(F-ET)的286例病例,超声引导下胚胎移植时,测量宫颈内口至宫底的距离及含胚胎的强回声光点距宫底的距离,并计算胚胎至宫底的距离占宫腔深度的比例,根据胚胎距宫底的距离占宫腔深度的比例不同分为A组(〈1/6)、B组(1/6-1/3)、C组(1/3-1/2)、D组(〉1/2),分析比较各组间妊娠结局差异有无统计学意义。结果冻融周期中A组、B组、C组、D组间一般情况及治疗情况比较差异均无统计学意义(P〉0.05);A组、B组、C组、D组间,临床妊娠率、胚胎种植率差异有统计学意义(P〈0.05),流产率、异位妊娠率等差异无统计学意义(P〉0.05)。结论胚胎移植过程中,由于宫腔深度的不同,胚胎距宫底的距离占宫腔深度比例影响妊娠结局。
Objective To explore the effects of transplanting position in the process of freezing and thawing cycle on pregnancy outcomes. Methods Retrospective analysis of 286 frozen thawed embryo transfer(F-ET)cases in the first hospital of Shanxi Medical University from September 2014 to July 2015. All the cases were employed to measure the distance from intracervical mouth and contain embryos of strong echo spot to the bottom of the uterus when embryos were transferred guided by ultrasound. According to different ratios of the distance from the embryo to the bottom of the uterus to the depth of the uterine cavity,transfer cycles were divided into group A(1/6),group B(1/6-1/3),group C(1/3-1/2) and group D(1/2),analyzing if there were significant differences in pregnancy outcomes among the four groups.Results There was no significant statistical differences in the general situation and treatment condition among group A,B,C and D in the frozen thawed cycles(P〈0.05);among group A,B,C and D,the clinical pregnancy rate and embryo planting rate were significantly different(P〈0.05),and there was no significant differences in the rate of abortion and ectopic pregnancy(P〈0.05). Conclusion In the process of embryo transfer,due to the difference of uterine cavity depth,the ratio of the distance from the embryo to the bottom of the uterus to the depth of the uterine cavity influences pregnancy outcomes.
